I had a clear Ga 68 PSMA PET but the 'better' Ferrotran nanoparticle MRI (sadly still very limited availability in Europe) successfully identified cancerous pelvic lymph nodes - confirmed by surgery. There can be multiple reasons why scans miss mets, including size and contrast agent uptake. Some radiologists suggest trying differing contrast agents in hopes one will work. Hope this helps. Bone scans aren't very sensitive.
I had this scenario for 2 years. MO focuses on scan compares and lack of radiographic progression and tends to ignore a rising PSA which is viewed as just a biomarker. Personally I was concerned about the level of granularity of those scans but that is SOC.
Why hasn't your MO prescribed more sensitive scans? I've had Axumin and Pylarify scans. They both came back negative for my slowly increasing PSA. Prostate is still intact. I am 74.
