MSK no longer offering Transperineal Biopsies

I called MSK and was advised they no longer offer this service for new patients.

Does anyone have recommendations of other USA locations and specific Doctors, looking for the providers who have had the most high volume procedures done under IV sedation (Not just lidocaine) and completed successfully with good experienced prostate expert pathologists.

Did MSK give any reason for discontinuing transperineal biopsies? Reading the link allowed me to imagine the one I had. I was wheeled into the OR and put under Propfol IV by a group of interns. After I was prepped the doctor came in -- I never saw the doctor before or after. Later learned the procedure was done by one of the interns under his direction.

Of course, I don't begrudge the intern (lady) at all. I was honored to serve. But the doctor should have told me, and seen me afterward.

I never saw this doctor after he read my MRI and agreed to do a perineal. After I was the unwitting star of the perineal class, I had to call his office to find out what came next. That is when the receptionist looked up the procedure and said Chrystal had done the job. The doctor's follow up was a 15-minute phone call where he gave results of the biopsy and the choice of his surgery or radiation elsewhere. I hardly understood anything at that stage. I still wonder how good that biopsy was, and if it affected my Gleason score.

Well, the person who I spoke to was very nice, kind and helpful, no rush and no problem with whoever I spoke to. I don't live there in NYC but due to their reputation and a referral of a friend, hence why I wanted to go there. I asked in several angles/ways but failed to get a real straight answer. I suggested that perhaps it might be because of high demand and since I am not a registered patient using their registered physicians (I called myself, no referral), and got no response to that questioning angle. I also asked if it was because I didn't have a physician or ask to join the entire suite of services of the program of treatment, just the biopsy, but no response to that either. I did mention it was Medicare though, also no comment received. What I was told that they discontinued it as of over a year ago implying no more "outpatient" admissions for this procedure. YMMV. Maybe it's a short term business decision? No idea, never navigated the medical system in the past 21 years.

Seems more business or insurance related than medical. Perineal does require an OR setting, and an anesthetist. The procedure takes a little more time, and there is the recovery period from the Propofol. And the link that was posted described the difficulties that can be encountered with an unconscious patient.

Compared to the simplicity of the rectal that my urologist's PA told me would be done in the office with nothing but Lidocaine jelly. But I thought most medical centers were gravitating toward perineal as safer.

I have had two Artemis MRI Fusion TP biopsies with Dr. Uchio at UCI Medical Center. Both biopsies were a piece of cake and took about an hour long each and I was awake the whole time. He has been doing TP biopsies for about 10 years now.

So you were only on lidocaine and not Versed under IV sedation?

I was awake the whole time both times - which each TP lasting about an hour. He injects a nerve block on both sides prior to the procedure and I didn’t feel a thing. Minimal blood in urine and semen for about a month.

When I had my biopsies, 2000 and 2002 I didn’t even receive lidocaine. Each needle was more painful. The first was only 6 needles. The second was to be twelve but I brought things to a halt after ten.

A strange decision --' Did they say why , Was it a cost issue ?

Transperineal is fast becoming the Gold Standard ( excluding the emerging In--Bore or Gantry - Real Time Biopsy .) in countries worldwide . In fact some countries will no longer perform Transrectal Biopsies because of the higher risk of infection - potentially deadly Septis .

It's an insurance issue .

Like many of the other replies . I had two Transperineeal MRU Fusion Biopsies at an outpatient off - site clinic . ( no Hospital ), at age 84 + I was awake all the time , . In and out in under an hour . A walk in the park .

I had an usual procedure regarding my Transrectal Biopsy. Prior to the procedure the surgeon brought in 6 female interns into his private office and asked each of them to balance a wooden cane on their middle finger. When I had the Trans Bio it was performed by one of the interns (no issues). So when the Doc came in to see how I was getting along, I asked him if the intern he picked for the procedure was best in balancing. He replied "no, I picked her cause she has the biggest tits......
