Similar experiences and outcomes when cancer ‘wakes up’ about a year out from Docetaxel? PSA slow rise, now 1.9

Hey,

Looking for hope…aren’t we all?

Husband had good results from Docetaxel, obliterating many bone mets and lowering PSA. Had CT/bone scan a couple months ago, showing suspicious areas. Dr. Ordered PSMA PET scans. Went over results with Dr. George today. He took my husband off Nubeqa due to side effects and ‘it doesn’t seem to work on him’. (Sounds like castrate resistance has kicked in?). Will get a port soon and begin Cabezitaxel(sorry if I spelled that wrong).

Anyone have similar experiences and know of future hopeful options for these conditions? Immune treatments are not an option since my husband takes immune suppressants (likely the reason this cancer roared into our lives).

It is my personal opinion,

Please do not stop any treatment that doctor recommendations. I have some questions for your doctor

I would ask this questions to your doctor

- Cabazitaxel, pluvicto or xofigo - Can we try with Pluvicto or Xofigo and keep the Cabazitaxel for future

- Can we do radiate to high mets area than do target therapy or chemo ? Many times high mets area do not response to chemo and target therapy so lets kill them first and do systematic therapy

- Can we have options to enroll in the clinical trials like AMG 509 or ADC and keep other FDA approval medicine for future ?

Questions for you

- Does your husband has any bone pain ? if yes than radiate them immediate many times oncologist do not refer you radiations oncologies so please see radiations oncologist

Dr George at Duke is one of the best! I hope Jevtana works as well as docetaxel did.

Any discussions about radiopharmaceuticals (Xofigo or Pluvicto)?

We love Dr. George! Yes he mentioned Pluvicto for 2 of the spots but we are hoping the Jevtana will wipe them out plus all the ‘pink’ areas Doc said is cancer diffusing into the tissue.

my husband has had 2 different times with taxotere chemo (2018 & 2022). With other treatments in between. After 9 rounds in ‘22, his PSA started doubling. PSMA scan showed Mets in spine, pelvic bones and left femur. He tried Pluvicto as he was an excellent candidate. But after 4 rounds his ALP and PSA was up over 230. A scan showed major uptake in bone Mets, skull to femurs. He is now on Cabazitaxel. Has had 5 rounds. PSA 59. Feels pretty good but fatigued day 6-15 each round. Tries to walk at least 20 minutes each day. He is 71. We are on year 8 battling PC

Thanks for your reply! Was your husband stage 4 at diagnosis? 8 years! Whew! My husband is also 71. I’m so glad the Cabazitaxel has driven the PSA down this far, hope it goes all the way down and stays there! Thanks for stating how he feels on days 6-15…this is good input for my husband due to his work schedule, maybe he can plan around those days. I feel for y’all! You know this roller coaster well. Hoping for the best for both of you!❤️

My husband was told surgery would be one and done. No scans were done in 2016-17. So he had surgery and then 35 rounds of radiation. But PSA didn’t go down so we switched to a cancer center. Found Mets in the spine, stage IV aggressive

Hello, I was not that familiar with Cabazitaxel, but keeping with my motto"You can learn something new every day", I found this online.

Sold under the brand name Jevtana, is a semi-synthetic derivative of a natural taxoid. It is a microtubule inhibitor, and the fourth taxane to be approved as a cancer therapy.

Cabazitaxel was developed by Sanofi-Aventis and was approved by the US Food and Drug Administration (FDA) for the treatment of hormone-refractory prostate cancer in June 2010. It is available as a generic medication.

Cabazitaxel is indicated in combination with prednisone for the treatment of metastatic castration-resistant prostate cancer following docetaxel-based treatment.

Mechanism of action

Taxanes enhance microtubule stabilization and inhibit cellular mitosis and division. Moreover, taxanes prevent androgen receptor (AR) signaling by binding cellular microtubules and the microtubule-associated motor protein dynein, thus averting AR nuclear translocation.

Clinical trials

In people with metastatic castration-resistant prostate cancer (mCRPC), overall survival (OS) is markedly enhanced with cabazitaxel versus mitoxantrone after prior docetaxel treatment.

Sounds like you have great Oncologist on your side and promising results in the future, hope others on this drug will chime in.

BTW Mr. Tall Allen gives excellent advice!
