Cyberknife+IMRT combination treatment for High Risk PCa, that has spread to the seminal vesicle.

Has anyone done a cyberknife-imrt combination radiation treatment for prostate cancer? My PCa has spread to the Seminal Vesicle. So, instead of a planned 5 session cyberknife treatment to the prostate only ( when the PCa was thought to be confined to the prostate), my doc is putting me on 3 cyberknife sessions ( 20gy) followed immediately by 21 sessions of whole pelvic IMRT (42gy) to deal with any microscopic spread. PSMA scan showed no spread beyond the seminal vesicle. He says the combination will deal with the prostate, seminal vesicle and any surrounding micro spreads that are too small to be detected by the scan.

Has anyone gone thru this? What were the side effects ? how effective was the treatment?

It's experimental. But SBRT can get the biologically effective dose as high as brachytherapy.

Hope your treatment has been successful. My husband is having a similar treatment regimen like yours, 5 cyberknife sessions followed by 25 IMRT. His SBRT is given as a boost and IMRT as a prophylaxis for pelvic lymph nodes. He will be finishing up his last SBRT this Friday and then move to IMRT. He has been experiencing minimum side effects so far, but, as we read, they typically show up one or two years down the roard. We will keep our fingers crossed.

Thank you. Wish the very best for a full cure for your husband. I am on Flomax - it is helping me deal with the side effects. Hope to wean off of it in the next month or two. Rgds

Thank you! My husband tried Flomax but it caused a very low blood pressure. So his doctor just prescribed Silodosin (Rapaflo) to see if he will tolerate it better. Wish you the best luck with your treatment too!

Thanks for asking. So far, so good. Continuing with the ADT regimen (Orgovyx, Abiraterone, and Prednisone) and daily dose of Flomax. PSA level is still zero. I get the blood work done every two months. No noticeable side effects due to the ADT. Getting tired easily though; have occasional hot flashes-that number also has reduced considerably now. Forcing myself to either go for a leisurely 2mile walk everyday and/or go to the gym for about 10 mts of treadmill work plus low weights on a set of machines and/or 30 mts of slow swimming. Of late, feeling the cold weather in my bones; body and bones ache more. Routine gets interrupted when I travel (especially when going on long distance flights (during which I miss a dose of ADT when time zone change is close to 10 hours)). Have been on ADT for 12 months thus far (including pre-radiation prep time-adjuvant). Plan to continue the ADT for another 12 months at which time would have to decide whether to taper/stop. Hoping that PSA remains at zero. Plan to get a PSMA scan, depending on PSA score, later this year. Plan to taper Flomax since the need for it seems to be reducing. Praying that the radiation plus ADT have done the trick; hoping for research breakthru’s. Actively reading all the posts in this site! Planned to post when there was a change, when I reach a decision point--hence silent thus far. Thx
