Just had my first erection in 4 years after 4th injection of testosterone. Very disappointed in the decrease in size as I am literally 1/2 the man I used to be (lol). I am a good researcher but wanted to find out if anyone out there has had or knows someone who has had any success with pumps or any other therapy to increase the size of their pride and joy.

I hear you, Brother.

I lost a good two inches in length after my RP back in 2016, which isn’t surprising considering the amount of urethra that was lost inside the prostate after it was excised. The urethra doesn’t stretch, so you have to make up that length somehow.

I don’t respond to PDE5 inhibitors, only Trimix, which persists for several hours, with a gradually increasing dull throbbing ache.

Please tell me more about your testosterone injections and the effect it had on your erections. I’d give my eye teeth for that! I’m 62, BTW.

Have you noticed any side effects from the T?

After my 2nd recurrence in 11 years and 2nd radiation session for those recurrences I broke down and went on Lupron for 2 years. T never returned and energy and focus never did either so I decided to try T Replacement. After 2 weeks I started looking at females again and after 4 weeks I had oral with my wife (yesterday) and had an orgasm. Freaking miracle. Being on T replenishment requires careful and frequent observation of PSA and blood. I feel a little more energy and hoping for more when I hit the 3 month mark. According to my surgeon, my bladder was pulled down to meet the cut in urethra.

The prostate was 1.5”, the net loss was .5”.

Fair enough. I was never told about the atrophy of penile tissue before surgery. Shortly after my surgery, on a follow up with whom I thought was the surgeon, I met with someone who presented the vacuum pump to me, told me how to use it, when etc. It is the Osborn Erecaid Esteem with tension bands ($250). I used it for about a year but just found it to be too much bother. Yes it helped restore (more width than length, but a little of both and some feeling) I also bought a Viberect medical vibrator for ED treatment ($300). All this helped somewhat with the use of viagra. I did sustain nerve damage during surgery so expected nothing.

Thought about a penile implant (Please honey, wait a minute while squeeze the bulb between my nuts 35 time to get a partial erection)

Now I will probably just sell my stuff on FrankTalk.org.

One of my pet peeves too. The atrophy and the boobs. Urologist treated the disease but not the person. What the hey, wasn't his problem.

Best medicine I found was Levitra and a woman who understood, and helped, my wife (who passed - sadly) and then a gf. I also had significant loss of length from the RP (in 2012). Penetration the major issue. Then just 'feel'. Erections not a major issue with Levitra, but had good nerve sparing. Nausea from Viagra a shocker. Went Levitra and better, but not brilliant.

With consistent effort, time and medications function may improve, erections may last longer, libido more robust. If not there are still good options. Injections or an implant can be good choices.

The actual size you’re left with is permanent. Onward!

Great to hear from you Larry! We are certainly old timers in fighting this thing. My new thing now is narrowing if the ureters caused by the radiation. Having surgery for reconstruction on the left ureter next month. Right one to follow later in the year. All you can do is keep on keepin on. Take care friend.

That's what the urologist should have suggested but didn't.

I had similar issues following my RALP last August, I have had some gain with the RestoreX device. restorex.com/. Its expensive but it is a serious piece of of equipment that is designed for men who have undergone radical prostatectomy. Together with the vaccurect and a bathmate hydro pump, I substantially recovered a satisfactory erection (not perfect) and some of the length that I lost from the procedure. Hope this helps.

Further info regarding VEDs (pumps) that might be helpful, based on my use of a VED following RP: The urologist who performed the RP, and the physician's assistant specializing in sexual function who works with him, recommended use of the VED daily beginning 8 weeks after surgery, in order to avoid loss of elasticity. Use of the VED was somewhat painful the first week or so, which in retrospect might have indicated some recovery of size. However, I did not take any measurements.

It seems like folks in your situation could get a prescription for a VED , so that its cost would be covered by medical insurance. I am satisfied with the manual Encore Medical En44014 Revive Custom Manual Vacuum Therapy System, which appears to no longer be available via Amazon, but available from other sources.

Beware of what the VED sales folks might try to push onto you: The medical group arranged for a session with a VED representative from Timm for their PTV and Osborn systems. She agreed that they would submit to my insurance the manual Osborn ErecAid Classic ($159), which appeared good enough for me. However, when their central sales rep finally followed up with me ~2 weeks later, she told me that the only device they would submit to insurance was the ~$299 system with battery-powered pump. I was especially irritated that their delay in replying to me was delaying start of its use, so I searched the web and Amazon and settled on the one described above and paid for it from my health savings account, expenditures being below the deductible limit for my high-deductible medical insurance anyway.

I found that I could multi-task, shaving or flossing & brushing my teeth during VED therapy. That also helped alleviate my wife’s concern that I might be enjoying the VED therapy.

When you say pelvic node that you were radiated at, what part of the pelvis was that?

Are you doing the T addition, as a do it yourself BAT, or is an inconsistent guiding your T usage?

Are you presuming that the cancer is gone for the long haul?

What do you do for blood flow increase. I’ve never stopped getting night bonors since I started adt in 2016 but it doesn’t stop the overall body shrinkage. I had imrt and brachytherapy in 2015, and since then my dick has gone from 8 in, down to barely 5in. (I’m Irish too) It’s shrunk in thickness too, so it’s not just from the urethra shortening from surgery. It’s also from the lack of T. Without the T, I have no desire. As a bachelor, I’ve been dating women my entire life but now all I do is talk with them like friends. It’s one of the many, very sad effects of all these adt. And I’m not convinced that the adt is worth it. It might extend my life by two years or so, but at what cost? Since 2015, my vibrant life, sex, travel and everything else, has become a hermits life, with very little excitement or fun. I was playing in the 20 year olds ice hockey league, playing in an ultimate frisbee league, basketball, tennis, going to concerts with different women all the time. Now, I barely do anything other than shop for healthy food, take pills and watch the TV.

The shrinkage is similar to other parts of my body, especially my neck, legs etc. Just about everything has shrunk and narrowed, even with proper eating and exercise (cardio and weight lifting).

I am curious whether other organs inside the body, have shrunk as well. Have you read or heard anything about that?

ADT switched off my desire completely. Testosterone turned it back on and now I feel the same as I did before ADT. My PSA is 0.006 so not cured but the lack of energy and focus is a shitty enough QOL to try the T restoration therapy. The PCa recurred in my right iliac.

Prostate removed in 2010 and was non-detectable for ten years until the beast resurfaced with a vengeance 3.5 years ago. Several rounds of radiation and Lupron temporarily destroyed my desire but still managed to have 3/4 wood sex with no orgasm. Despite my doubts, my urologist insisted a pharmaceutical grade vacuum pump would work wonders. Combined with a 100mg Viagra not only can I now cut diamonds, but the pump has restored my original size. (actually had to buy a larger size tube)

Libido is still weak but I'm able to reach orgasm. Looking forward to ADT vacation in a few months and T returning. But after two years of Orgovyx and Zytiga who knows what will happen. Age 71 and very active lifting weights, hiking and wrestling. Training hard and sleeping well is one of our best weapons.

I am so sorry that like me you were told NOTHING about atrophy of penile tissue after receiving a RP...it astonished me after a year when I found out what was happening. Yes, your penis atrophes, i.e., shrinks! To figure...ya think this would be an important thing to tell men when they are being groomed for surgery! But I find that its not mentioned; guess Dr's think 80% of us would choose not to proceed.

I attach my 'woulda-coulda-shoulda' list of concerns, revelations and discoveries I made in my journey if it can help anyone. I caught my atrophe 1 year post RP and used a pump systematically to maintain what was left...I still lost size and am down 1/3 of what I was...it will bug you, especially (hopefully) we realize there is a good chance we can expire with this disease not from it! But NO ONE tells us ANYTHING about this issue until its way to late to save what you were born with...or at least, as much as you can.

So, yes. The pump can help start to stretch and grow what remains...at least it wont keep shrinking...after a RP you dont have nocturnal erections which strecth and nurish the penis...it starts to shrink right after those stop...then if you used ADT that will destroy you ability to get natural erections; venus leak follows as close as 6 weeks post ADT star...AGAIN none of this is explained.

If you can get natural erections work with that as long as you can. Try to stretch and recover what you can; you will never get back what you have...at least I dont think so...but you can stop what has happend and at least grow it from here.

I choose to have an IPP (iflatable penile prostesis) put in a 2 1/2 years after my RP; wished I had done it right before my RT (salvage radiaton). My natural erections were gone for good but no one felt the medical

I had 3/4 erections two weeks past surgery--even lost half of a bundle. And been successfully using Viagra ever since. In fact it's critical to use Viagra or similar, every night right after having the catheter removed. Anything to keep blood headed south to avoid atrophy. Some doctors now recommend Trimix after surgery as that will wake the dead with or without nerve spared.

Some guys wait too long to inflate junior and that just makes things more difficult later. Use it or lose it.

Guilty of waiting too long. Didn't give a shit when I was on ADT; I even knew it was happening. 1 month on testosterone replacement and all desire is back. Just like turning off and on a faucet. Now I want my boy back. It's 90% hard but it's barely there. Thanks for the information!

I was also super pissed my surgeon did not tell me I would lose physical inches when the prostate is removed and the void is reatached. I confronted him on it but he said there was nothing that could be done. I just wanted him to prepare me. I am over it now but then 4 years of no erection including 2 years of ADT and the following 2 years with no T recovery has stolen another inch or 2. Big difference between a 6-7" normal penis and a 3-4" penis. Literally am embarrassed every time I have to expose the little guy to a medical professional. Going to try the pump and pills and see I can recover some size. Thanks for the response, very helpful to hear your story.

I totally get it Mopesy, and it looks like many others do as well. I was really angry at my medical team for not giving me any info. Thank God I found an old post by TA that pointed me in the right direction, but at that point I had already experienced atrophy.

Took a while to get over seeing my diminutive penis in the mirror. When I finally got an appointment with my uro to discuss trimix, I was worried that there wouldn't be enough shaft to be able to properly inject the needle. It got so bad that occasionally my penis was an "innie" instead of an "outie."

Fortunately, with a lot of rehab I can get an errection again. Nothing like what I used to have (which wasn't going to win any size contests in the first place), but enough to achieve penetration.

Understand the innie/outie experience. Through RP and 2 recurrences that required 38 radiation sessions each plus 2 years of Lupron I have paid my dues to desire something more than just my life. Not that I'm not grateful for life. Once my T fully recovers I will put my focus on gaining some size. Thanks for your response.
