Recently I have had a couple of molars break off . I've only had a treatment of Lupron for 6 months . Two each 3-month injections. Could this be related to the Lupron along with the other side effects I have enjoyed like hot flashes, fatigue, muscle pain, and weight gain or just coincidental?

I have been at this battle for 76 months , and my teeth are falling apart. How can I get teeth pulled when I am doing Zometa?

After 54 months of Lupron - Xtandi, I have two molars broken off mostly at the gum line ( OK since they were root canals ) and a couple front teeth with new cracks or little pieces missing. Teeth were perfect before I started ADT. They still function and I eat with them just fine. No biggie . Not worth the thousands to get every thing fixed properly.

yayahahahaya yayahahahaya. I love it, man you live in a way different aPCa world than I wished I did. I can only imagine / dream thinking about things like my appearance and those fine ladies like you can. I get so much Zometa that “ any “ mandible work would be sure necrosis and metal plate implant. I hate dental work anyway. I’m envious… maybe if I got some skin wrinkle cream and lost a couple of pounds … dunno. Maybe a tux would spruce up my appearance. Just say’in Yayahahahaya.

It most likely is related to Lupron, one of its many SE's.

ADT causes increased osteoclastic activity,6 bone resorption,7 bone loss8 and an increased risk of fracture.9 The rates of bone loss are 2% to 8% in the lumbar spine region and 1.8% to 6.5% in the femoral neck during the initial 12 months of continuous ADT.2,6.

Periodontal disease results in resorption of alveolar bone and loss of the soft tissue attachment to the tooth.

I just consulted with Dr. Google and Lupron, Prednisone, Prolia -- all list possible dental problems as LT SE's. Of course our dental health declines with advanced age too.

I have been on lupron since 2016, every 3 months. My Doc did a bone density test April 2022, they put me on Alendronate Sodium 70mg to help on my bone lost. Maybe check with your Doctor about that.

I have been on Zoladex for almost 2 years, and Zytiga. To help the bones I take AdCal-D3 twice daily. Also Alendronic acid following a Dexa scan showing ostopenia (-1.3 in femur). So far no problem. Good luck.

I am a dentist is Australia. Certainly the bone surrounding teeth can be affected and more prone to periodontal disease but it will not make the tooth weaker according to most literature . However the drug can affect the oral environment as with aging process so mouth is dryer and more acidic and making teeth more vulnerable to decay and breakdown/ erosion/ attrition of teeth.

Try to stimulate saliva with sugar free gum and artificial saliva like biotene helps enormously.

Use Tooth mousse by GC as a dental tooth paste also remineralises the teeth

That’s the first advice by a dentist I’ve read on the subject, thank you. All I know is like everything else, we also have to be fastidious about dental health as we age, regardless of disease, treatments etc.

My husband suffered with gum disease years before his diagnosis of advanced prostate cancer. On his diagnosis tests revealed he had low testerone. I believe his gum issues were a red danger flag for prostate cance but we were in the dark about it back then as no routine checks for men in the UK.

His teeth although crowded are ok, it's his gums that are the problem!

Good timing this. On Enzalutamide for almost 3 years and tooth just broke off earlier this week. I'm in the UK and my local prostate support group leaflet recommends using a toothpaste called 'Duraphat 5000'. This is prescription only though from the dentist. Advanced prostate cancer just keeps on giving and giving.

I’m the same teeth getting damaged beyond the norm. Getting them pulled and going dentures is my logic at this point.

Ive been looking at it this way after 5+ years on ADT. The repercussions from the therapy is stressful and I me personally have found stress is making me grind my teeth. Think about are you relaxed when going though a horrific hot flash? I’m sure not. I say some nasty words sometimes and generally find myself biting the bullet you might say all hard on the teeth. I’m surprised the dentist didn’t bring this up.

I’m no dentist but thinking at least a teeth guard at night would be helpful. If someone told me 5 years ago the stress would cause possible teeth grinding it might of stopped me from getting dentures.

Nonsense. Usually teeth break off because of neglect. Once a tooth is formed and erupted, only oral environmental factors can cause breakage. Decay due to dry mouth, xerostomia, poor oral hygiene and unrestored teeth after root canals, Endodontic treatment. Regular preventive Dental care in the Dental office would eliminate most tooth fractures.However periodontal disease is also the result, in most cases, due to poor oral hygiene, poor care in the Dental office, yes that happens. Bone loss can result from Lupron etc and usually a bisphosphonate, ie. Prolia, Xgeva etc. are given with 3 month Lupron Depot, or Eligard, to prevent bone loss and pain. Also Calcium+D3 will help bone strength. EXERCISE is a must.

When the bisphosphonates are given IV, Osteonecrosis of the jaw can result in a small % of Oral Surgery procedures. Patients are given that information before procedures.

It is advisable to have all Dental care done before taking the Bisphosphonates. Bisphosphonates slow down the Osteclastic process. A constant change in bone is always occurring. Osteoblasts lay down new bone while Osteoclasts take bone away. Hope this info is valuable. D.D.S.

Teeth break, they wear down, it is not the fault of the person. I have 3 molars out of four that are worn down to the level that I can feel it with my tongue or finger, but there is no pain yet. It is like someone sanded the last half of them off from chewing.

All good information. Interesting on the dry mouth as oxybutynin that I take for the hot flashes makes my mouth really dry. And I definitely have been grinding my teeth more or at least clenching them.

Oxybutynin for hot flashes? No matter why you are taking this antimuscarinic, you really, really need to know about this from the NIH (emphasis mine):

"As far as bladder medications are concerned, immediate release oxybutynin has been shown to adversely affect cognition in the cognitively intact elderly. The newer antimuscarinics for OAB – darifenacin, fesoterodine, solifenacin, tolterodine, and the quaternary ammonium compound, trospium chloride – do not appear to adversely affect cognition. Oxybutynin transdermal gel similarly appears to have no demonstrable adverse effect on cognition."

"It is well-established that the use of medications with antimuscarinic properties (e.g., antidepressants, antipsychotics) is associated with impairment of cognition in community-dwelling older people. Cholinergic mechanisms are implicated in the neuropathology of Alzheimer’s disease and the continued use of anticholinergic medication over a four-year period appears to be associated with cognitive impairment and an increased risk of incident dementia."

Of course your doctor will probably discount this -- every urologist I've met has -- but you need to be the one who makes the informed decision on medications that the National Institutes of Health say can "adversely affect cognition in the cognitively intact elderly."

F-ing Lupron jabs! I highly recommend E2 gel! Only side effect is little boobs.

I refuse to subject myself to those nasty chemical injections!

Because the molar breaks happened after only six months of ADT, I doubt that there's a causal relationship. More likely, these molars were likely to fail anyway.

A thorough dental exam might be in order to (hopefully) prevent further problems.

Makes sense..dental apt is made. 6 months of fatigue, hot flashes, shrinkage below the belt, addage above, and all that, seems like an eternity. Not sure how all you warriors have done years.

I've been on Lupron more than 9 years and had two molars pulled in the past 18 months. Dentist felt that two teeth, both of which had root canals many years ago, (I'm 78) just wore out over time. I am having crowns put on two of the middle bottom teeth which are getting a lot of wear because I am using those cutter teeth as chewers with the two bottom molars gone.

I'm not aware of that being a result of ADT but I could be wrong. After 3+ years all my teeth are intact. What you have to be aware of though if you go on any bone-strengthening medications, you should first have all known dental work done because necrosis can result from the dental work.

4 Molars gone in 4 years and a 5th one on the way, after spending $5K trying to keep up I'm done. 😟 started after my 1st treatment with Zometa, discontinued the Zometa now just Lurpon/Xtandi combo, coming up on year 4 , they're cracking off , mostly at the base. Seems similar to others description here.

I use a mouth guard on top and bottom teeth. After 2 years on Eligard my teeth (and bone density) are still very good. A Periodontist I used to see told me years ago he sleeps with a mouth guard. At the time I thought that was overly cautious. I now think he is overly smart. I personally consider it irresponsible to not use a mouth guard now.

The mouth guard is a good idea and interesting. Sleep is also interesting. Not sure I remember what that is. Melatonin, sleep aids, soothing music, nothing really gets me more than an hour and a half sometimes 2 hours. Even with medicines to help with the intensity of the hot flashes The Hot Flashes wake me up. But I seem to have adapted to limited continuous sleep.. And at least for now they've only got me on this 6 month ADT treatment post surgery and radiation so maybe all that will start to get better over the next few months.

I urinate every 2 hours so sleep for me is an interrupted 6 - 7 hours.

Try three mouth guard much much better than the alternative
