I've been watching my PSA since 12/1 RP, while contemplating the next move. A recent PSA result was .173. A Pylarify PSMA PET/CT showed a "small metabolically active left presacral LN .7 x .9 cm max SUV 3.1

My RO is recommending 6 months of Orgovyx concurrent with pelvic RT beginning at the same time and asap.

My concerns other than PCa are SEs of bladder issues, urinary incontinence and bowel issues.

Your suggested treatment thoughts please? SOC? How to mitigate SEs? What to ask the RO before or during simulation? RO hasn't provided Gy. What info is prudent to know before I start - Gy? # of treatments? Plan or image of what is going to be radiated?

You will most likely get pelvic fossa with lymph nodes.

Asking about gy was counterproductive in my case, as I was getting VMAT EBRT, and the response was at which position and angle!!!

You say you've had RP? They will work around the bladder and neck. Again depending on the type of EBRT

Today was day 1 of 33 for me, same situation, RP done 4 years ago, now a hot lymph node.

Similar to my story. Started Lupron and Abiraterone + prednisone for 4 weeks. Then, i received 33 whole pelvic radiation treatments. I will continue on the drug therapy for at least a total 24 months. I completed radiation 8 months ago. So far, PSA is undetectable. Radiation wasnt that difficult for me but everyone is different. I definitely had more urinary urgency and a little bit of bowel upset, but neither were unmanageable for me. Both returned to normal within a couple weeks after treatment.

Did you not have incontinence after RP? Did you overcome it?

You'll overcome RT, just stay healthy and active!!
