First PSA test ever and concerning results

Hi. I apologize if this isn't the right board to post in but thought it might be a good place to start. Just had my frist PSA screening done and got borderline results:

PSA: 0.4

PSA Free: 0.1

PSA Free%: 25

Any thoughts on how concerned I should be about the PSA Free% of 25? A little bit of reading tells me PSA Free can be an important indicator of cancer risk and I'm right at the level considered low. Looks like there are some more aggressive forms of cancer that can have a profile with low PSA and low PSA Free so am a little nervous. Thanks for any thoughts or advice.

Edit: Sorry meant to say that I am 53 years old and healthy as far as I know

nothing to worry about unless youve already been treated

You are right, it isn't the right board. "Prostate cancer network" could be more suitable.

Coming now to your query, the PSA numbers you posted are very far from being indicative of a malignancy, for a treatment naive man that is. I assume you are treatment naive because after treatment freePSA is superfluous. Specifically, the 25% freePSA /totalPSA ratio doesn't say much due to the rounding error of the single place decimals. Not-rounded it can be anything from 0.05/0.35 ~ 14% to 0.15/0.45 ~33%. Retest again after 6 months and see how it goes.

Congratulations! And go enjoy your life. In 10 years, have another PSA test, and if it continues to be that low, you never have to have another PSA test again. % free PSA is not valid for PSA as low as yours.

You don't have to worry. Free PSA is concerning when is low, below 12 or 10.

if you didn’t get the finger in your rectum ask for your money back. Psa isn’t everything.

but I’m not kidding. My guy actually found the tumor then missed it with the biopsy. Think he called it a nodule. Not smart enough to get a scan. Push them for what you need.

PSA of 4 or more is concerning. Free % means nothing AFAIK.

Like a woman.........and in Pca........ a period is something to be concerned about. 4.0 is biopsy time, 0.4 which you are, is happy dance time...... So jump in the pool and swim your ass off......Past 49 years old Keep checking your PSA at least once a year. And that's a must for every male you know (family and friends)....

at 4.2 mine was out of the prostate and against the rectum. I’d say 1.5 is high enough to be worried.

You're safe if you're a woman (otherwise worry?)

NOTE: Always exceptions to the rule: (Remember, rule of thumb).

The PSA level in blood is measured in units called nanograms per milliliter (ng/mL). The chance of having prostate cancer goes up as the PSA level goes up, but there is no set cutoff point that can tell for sure if a man does or doesn’t have prostate cancer. Many doctors use a PSA cutoff point of 4 ng/mL or higher when deciding if a man might need further testing, while others might recommend it starting at a lower level, such as 2.5 or 3.

Most men without prostate cancer have PSA levels under 4 ng/mL of blood. When prostate cancer develops, the PSA level often goes above 4. Still, a level below 4 is not a guarantee that a man doesn’t have cancer. About 15% of men with a PSA below 4 will have prostate cancer if a biopsy is done.

Men with a PSA level between 4 and 10 (often called the “borderline range”) have about a 1 in 4 chance of having prostate cancer.

If the PSA is more than 10, the chance of having prostate cancer is over 50%.

If your PSA level is high, you might need further tests to look for prostate cancer.

Is the PSA

0.4 or 4.0??

Swimjim00 profile image
Swimjim00 in reply to

0.4, but the free % was 25. But someone commented above that fortunately free% isn’t relevant when PSA is that low. Thanks

I don’t get it.

Only a person that has had prostate cancer, and has been treated would worry about 0.4
