Are there still many options after being metastatic and now Zytiga and possibly xtandi failed- did this happen to anyone and now doing well? Also did anyone have psa ever go up on xtandi and scans showed in remission - ( does have prostate )

I am in a similar situation, just waiting for the Xtandi to fail. I am filled with dread when I read of clinical trials being a way forward. That implies that tried and proven treatment options are few. Please tell me I am wrong.
I feel the same way And hope someone disagrees with us. I feel like clinical trials are last resort. I’m praying xtandi works for a long time.
As I said, many options for mCRPC in the US - but fewer in the UK. Aside from abi and enza, these options are currently approved for various indications: docetaxel, cabazitaxel, Provenge, Xofigo, Pluvicto, 3 PARP inhibitors + abi/enza (maybe as of this month). Clinical trials are ongoing for several drugs used earlier, and for combinations one wouldn't ordinarily get.
if my dad psa went up on Zytiga but in the cat scan there was minimal change and the change there was - was in previous Mets- does that necessarily mean the med was not working-
That doubled in 3 months so you would check it again I guess in 3 months and if it goes to 2.4 there's a possibly a problem sometimes it jumps and goes back down just my two cents
they are going to do a PSMA scan- do I guess that's be cautious
Xtandi helped for a couple years for my husband. Switched to apalutamide, but that also failed big time. PSA kept going up. Scans showed some improvement on a few mets, but increase in many other areas. As you will see from the replies there are still options for you. Every treatment varies in success for everone. There is a lot to consider when determining treatments and your doctor know your case best. What worked for one case may not for another. Of course, ask your doctor why he recommends one treatment over another, he has his reasons. Our doctors always explained why they chose one over another.

I went through Xtandi and then Zytiga. My PSA climbed to 40ish and I went through chemo with Jevtana every three weeks for a year. PSA got to around a hundred. I was then given six monthly injections of Xofigo (radium 223), which ended about a month ago. My last PSA back then was 338. I am back on Zytiga and now lynzarza. I do not have a recent PSA. I have tolorated all of my treatments very well. My only complaint is being fatigued. I meet with my oncologist in a couple weeks to see what's next. My radiation oncologist is setting up a consultation with Barnes about another radium isotope. I have been continuously treated for 20 years...Lupron surgery, radiation, etc.
