Darryl, this is the single best description of the state of the art of PCa therapies that I have seen. It is what I had wished I had known when I first started discussions with my MO. And I would have to say, by some internal bias that I would attribute to the fact that all the answers are not known and only current MOs are up on all this, that I would want my doctor to be a practicing PCa oncologist rather than a urologist or an oncologist who doesn't specialize in PCa. But regardless, this was a most excellent presentation of the options available and why they would be recommended for XYZ personal patient situation. Thank you for putting this interview together, and thanks to Dr. Marshall for her part.
One of the things that concerns me about this forum and others is not that they aren't a tremendous asset for men with advanced PCa, but is that many who are being given a particular therapy don't really appreciate why another MO might not recommend a particular therapy that someone else, or multiple someones, on this forum would say you have to be on, for example, abiraterone rather than the one that their MO has them on. Or how the side effects of a particular therapy present to that individual, or comorbidities of that Individual, would lead an MO to select one therapeutic med rather than another at the particular "stage" of the disease they are at. This would seem to something that every man with PCa would understand. It is obvious that is not the case, especially for a field of practice that is changing fairly rapidly.
I do believe that not all MO's, even practicing PCa oncology specialists, are as up to speed, or would make the same recommendations that another MO would. I have personal experience of this. My MO did not, in my experience and opinion, fully take in to account a serious comorbidity that I have which, had I known, I would have refused the med I was given. This video does not get in to the level of detail that would have led me to know the details but it would have given me a better understanding of how the process should work.
Dr. Marshall did an excellent job in discussing the current therapies available and SOP for the different stages. It was very clear and just made sense. I, for one, have been confused and was confused about how the different choices are made for mCRPC. I definitely would be very happy to have Dr. Marshall as my personal MO.
