Pluvicto with Small Dog

I'm told that I will start Pluvicto in 1-2 months; I have a small dog. What is the guideline(s) to be following in keeping the dog safe? Thank you!!

I wonder, if I would have to have it down the road, my one 15 year old cat has a form of cancer in one paw and maybe the radiation would kill his cancer also. He sleeps at my feet and spends not so much on my lap, just a weird random thought.

Pluvicto is specifically targeted to prostate cancer.

I asked about this, and they did not have set restrictions for pets.

If the dog is younger, I personally would follow the more strict recommendations given for keeping distance from children and/or pregnant women: limit close contact (less than 3 feet) with children and pregnant women for 7 days, and sleep in a separate bedroom from children for 7 days, or from pregnant women for 15 days.

This is probably an excess of caution, and the degree to which one might adhere to it is of course a judgement call or educated (or uneducated) guess!

I had Lu 177 PSMA treatment in Germany in 2016. They kept me 3 nights in the hospital and then they released me without any restriction.They did a gamma camera study the following day after the infusion. If the radiation emmited is OK (depends on PSMA expression and number of mets) they allowed people to mingle in the recreation room.

I was out of my room after 24 hours and spent most of the time with other patients and after 72 hours I was free and back to normal.

The cancer was castration sensitive and the mets were in the lymph nodes extending from the mesorectal nodes to the retroperitoneum up to the renal arteries.

One treatment with Lu 177 PSMA was enough to made all these multiple mets PSMA negative and they have remained PSMA negative since the treatment. I had 5 PSMA PET/CTs since 2016.

I did not have major side effects. Some fatigue, short time edema because of all the amount of liquid the gave me.

No problems with the salivary glands or the kidneys. Creatinine did not change after the treatment. No neuropathy, nausea or hematoloigical problems.

Such good news. I am castration resistant and have just completed docs attacks all and Sophie go. I do have neuropathy in my feet and Mets in my bones so far. I thought l u l u 177 could not be used if the cancer was in the soft tissues . Is that true Thanks Tom

Lu 177 PSMA is a systemic therapy and treats the cancer in all the tissues if the cancer expresses enough PSMA (SUV around the liver). This includes lymph nodes, bones and visceral mets.

Lu 177 PSMA is also effective in mCRPC (Vision, and TheraP RCT ). Some consider that it may be more effective than chemo.

The treatment which can't be used with visceral mets is Xofigo.

Thanks for the info. I have been at this for 20 years. The Lord has blessed me With a slow growing cancer. The last two years can has raised its ugly head and I have gone through chemo, zofigo and now Lulu 177. I believe I start in January. My PSA is 33 and my last alp was 82. I know how upset dogs can get when denied their favorite human. When I was traveling with radioactive seeds implanted, there were similar warnings about children pets etc. I bought a sheet of lead (pretty thick, the Geiger counter screamed without the lead and was reading essentially background when on the other side of the sheet. My dog did not mind lying on the floor in the airplane so I never had to use the sheet.

Do not depend on lead shielding without testing with a known good Geiger counter (mine is actually a scintillation counter, whatever the difference is) I have checked my Geiger with watch hands, a small uranium ore piece (low grade) and also myself when doing PET scans.

I also brought the counter into the room where I had IMRT thinking I could glance at it across the room and see how much scatter there was. I wanted to know how focused the electron beam is. well... I forgot to turn the alarm off so it squealed for the duration of the treatment. There is a lot of scatter which is why they hide behind lead. I was nicely told that I did not need to being the counter next time

Mayo told us that there are no restrictions with pets. Our dog is 10 pounds. She sleeps with us and they said that is fine.

a small dog has same risks as human from residual radiation exposure. For a 10 lb dog you should follow the guidelines given for children.

No sitting on your lap, leaning on you, no sleeping with you, no licking your skin, for at least 7 days.

We were told our small dog should not sleep with DH or sit on his lap for 3 days after treatment. She is an older dog (13) and it would take a longer time for her to develop cancer from exposure than her expected life span. Still, protocol says no close exposure for 3 days.

Risks to an adult dog is very low. Most of the radiation (isotope) goes out in the urine in the first 24-48 hours. So just keep the bathroom door closed, clean up any drips, and flush twice. Please don’t exclude or deny cuddling or affection after first few days at most.

Tom, I had a different delivery system (ligand) for Lu177, a monoclonal antibody called J591. Unlike Pluvicto it is thought better for targeting low volume residual PSMA expressing PC such as micrometastasis not seen or with very low SUVmax. In combination with SBRT for oligomets I had two infusions in Australia last May. So far results appear very good. PSA down from around 0.24 prior to 0.048 and still dropping six months out. Next check in January. If it starts rising again will consider and discuss returning for another round of two treatments.

Details in my prior posts. As for dog and family exposure, with this ligand the strong beta radiation never gets outside of the body (not excreted in urine like Pluvicto). Only the lower energy photons (gamma) that are co-emitted. These are already measured and confirmed within “safe limits” within an hour after the infusion at 2 meters. Paul
