Yes, it has less effectiveness, of course.
I would ask the same question as smurtaw - why have you started on half dose? I started on Xtandi a week ago today on the full 160mg and though it's very early days, I haven't noticed any obvious change to the SEs I already had with ADT (these being mild hot flushes and some fatigue, but I work out 3 to 4 times a week which is keeping them under control).
The only thing I have noticed in the last few days is slightly increased pain from a couple of bone mets which is strange - could this be due to Xtandi? However, my CO just prescribed me a Calcium + D3 supplement so I'm thinking that is more likely the cause, so I've stopped taking it to see what happens (my last blood test showed Calcium was bang in the middle of normal range).
If I do start getting bad SEs from the Xtandi, then I will consider lowering the dose.
thank you for your response and for the information. I do hope the pain goes away for you and that you are able to tolerate the 120mg of Xtandi. I did read that the normal dose is 160mg then the doctor would reduce it 120mg and then 80mg.
I am on 80mg due to my age (86). My oncologist thought it would be best to start at 80mg due to the side effects. I’ve been on Xtandi for about 2 weeks now and only have a headache and fatigue. Maybe I should ask the oncologist to increase my dose?
My husband's oncologist increased slowly over 2-3 weeks from half to full dose.
I was taking the full dose 160mg for about 4+ years, my PSA dropped to undetectable and remained there during this time. For me however the side effects of it were cumulative and the fatigue, headaches and cognitive issues worsened over time. My oncologist, Dr. Sartor, recommended reducing the dose to 80 mg to help with side effects and said there is still good activity at the lower dose and he has several patients who do well at a lower dose.
I’ve been on 80 mg for the last 2-3 years and my PSA remained undetectable until about a year ago, and it remains extremely low. I asked Dr. Sartor if I should increase the Xtandi dose and he replied “no it’s unlikely to make a difference, don’t beat yourself up with it, there’s a lot of other options we can try once PSA gets higher”. So I’ve remained on 80 mg which definitely lessened side effects.
Keep in mind that 160 mg was the maximum dosage they were able to use when developing and testing the drug before side effects became unbearable. Many men do well on a lower dose, just follow your doctors advice, he probably has good reasoning to start you at the lower dose, ask him why during your next visit.

I was on the full dose and had no side effects. the Xtandi treatment lasted about 24 months for me until it stopped working. I am now on the Lutetium L-177 treatment schedule.
My father, 90 yrs old, lost appetite when on full dose of Xtandi. He has been on 1/2 dose for almost 2 years and PSA remains undetectable. Hope everything works out well for you.
I am on 80mg and it’s doing a good job having reduced my PSA to 0.006. Was on 160 mg but se was so awful that I had to reduce it to 120mg, still suffering and then 80mg. Been doing the reduced dosage for a couple of months now and having a life back. Every man is different, some can tolerate se better than others so don’t sweat over it. Just let your PSA tell you if 80mg is doing it’s job. If it is and with minimal se, why not?
