How do I access my records spacicaly all doctors entries.

Sign up to your hospitals portal. Like a MyChart, Patient Gateway and the you can access any doctors notes, after visit summaries plus all your medical appointments, test results. You can also link your different hospitals/doctors.The doctors notes are generally under visits then look at the visit you had then you’ll see the notes and after visit summaries.

The doctors notes have always been an aggravating thing for me. There are the “ hippa “ notes that are the notes you get for patient consumption… the ones everyone gets …called your medical record . But I always want to see the “ in house “ doctors notes that include the little colored “ flags , alerts , diagnostic thoughts for medical teams and other info ( grim forecasts, rehab alerts , financial changes … thoughts … and many more ) that doctors append to your in-house record. “ there are probably good reasons to not let patients access those, they might panic some and be misunderstood by others I suppose.

I'd like the same.

As you said could scare some people as it strays from bedside manner but for me would be interesting.

Side note on the scare part. I used to travel a lot in work.

United airlines had a channel on the armrest (channel 2) that you could hear the cockpit pilots, towers, etc. thru. Loved it.

The channel has since been Discontinued when someone sued United saying it freaked them out too much when a flight had a near air collision. Oh well.

I have been battling this demon since 1999. There is nothing in the notes that would scare me. Doctors error as they are human and I could help them treat me should I see an error . I find they are quite prone to over look things since they are so busy.

Most likely there would be “ stuff “ that follows you around …ie: cranky patient, frequently late or missing from appointments , challenges decisions, won’t follow blood sugar regulation properly, has opiate rehab history, shops around, tries to play doctors against each other … history of not following prescription directions properly , …. or one in my case, “ has technical medical knowledge/ experience, do not display xray films in front of him “ . In my case they don’t want to alarm me unnecessarily or repetitively with another reminder by looking at my films again. Doctors make educated and informed decisions that most of us aren’t skilled enough to make …. I might be freaking at seeing “ that damage “ again , they want me to have confidence in their decisions and not be freaking , taking it too seriously. I know I could easily go to records and get copies of all my films, but I choose not to just for reasons mentioned above. None of this type of info has to be , or even should be, in your mandated hippa records …it’s all internal notes for just the caregivers.

Nursing home , ICU , neonatal and many departments often have numerous appended caregiver notes …. No water 3 hours before bedtime, prevent him from seeking out sweets , check shunt every two hours…pt pulls out tubing frequently, etc…. stuff like this …

There are zillions of variations on this , and piles of potential topics , but this is the gist of it.

I knew a channel listening to air traffic control (ATC) communications would not fly ( pun intended). There always will be those that will make a mess of it.

No …. The in-house doctors , caregivers, notes are not required to be part of hippa. Many - times these notes are written in doctors shorthand so you can’t accidentally see and read them. Check out my answer to campsoups …above…

I just saw a doctor who took over a retired doctors patients. So he was kinda new to me. While I was in the room the doctor and his PA were ion the computer and the PA is saying see here as it was written notes. After that incident it seemed as though our relationship changed. I do want to read the information and correct it any whatever. The fact that I have been to Germany times and was able to get my demon to sleep now for 23 months this may be an issue with the doctor.

And it may be nothing . I usually am not a suspicious person.

See my “ example “ note below. And no you aren’t paranoid, it’s really very standard and common… After a while, like you, you can sense that everything is not alright.

I can give you a great example of how those “ doctors notes “ can screw you over and there is nothing that can be done about it. In my doctors notes I have a “ opiate abuse “ flag. Back 15-18 years ago I experienced horrible spontaneous debilitating panic attacks. Just sitting in a chair in the living room, they would happen and I felt I needed a ambulance to the E.R. I actually did take one ambulance trip.

The doctors thought my symptoms seemed like opiate abuse issues …. they tested me several ways and found nothing but sent me to rehab to clear the ugly symptoms anyway. 15 minutes with the rehab doctor and he knew exactly what it was. It was extreme sleep deprivation, which can resemble opiate withdrawal symptoms. I went to the sleep lab and I had a AHI of , jaw dropping, 87. Meaning I woke from sleep over 500 times a night, never entering rem sleep. meaning I never had actual sleep for several years. an autopap machine corrected it instantly , and I’ve slept like a baby ( better ) ever since.

The problem is that in my doctors notes I still have that opiate abuse flag that will never go away. This flag causes me to be treated like an addict by some doctors and causes me to have very close scrutiny and monitoring by my pain management doctor. I try to explain the circumstances of how that flag came to be and that it should be removed …. it’s never going be removed , no one checks with rehab to see that I wasn’t treated , and no one is inclined to correct the “ doctors shorthand notes that are appended to your in-house record.

None of this shows on my hippa record ( it is corrected there ) … but will remain on the in-house records forever and cause me unnecessary issues forever.

That is disturbing what happened to you.I wonder in the law is there a way to challenge this and have it expunged?

Oh well …. that horse got out of the barn long ago. I’m palliative care now and none of that matters much I guess . It’s a good example of how things can get screwed up. Yayahahahaya yayahahahaya you just gotta love it.

Thanks buddy .. I appreciate the sentiment and I hope you are as well. Everything is relative in this , sometimes, bizarre world …… in that context …. Right here , in the moment … I’m OK ….. considering. that’s a good thing .

My experience has been that many docs put all sorts of crap in their "real notes" that is highly questionable, if for no other reason than to justify coverage. Reading such notes gave me a better insight into their thinking.

Most doctors work for organizations or in offices with partners… organizations have productivity expectations.. and partners have huge insurance , front - back office personnel and building expenses ( partnerships help defray insurance cost ) … gotta pay those bills and collect those huge incomes that medical professionals went to 10-16 years of school ( plus in California, 50 hours of mandatory CME per annual license cycle ) … to be able to live the expected lifestyles

"MyChart" is a module in Epic's product EHR software, which is predominate throughout the industry. Cerner/Siemans would be another EHR software, so ask if your provider has an "patient" access feature.
