Jim's MO offered him Cialis. She asked if he'd rather take it "as needed" or "daily basis".

Totally out of my wheelhouse. Any thoughts?

I considered it and even had a prescription but ended up not even picking it up. If I go back on ADT I will consider it again for sure, for now everything is working fine. My thinking was to avoid a muscle from atrophying and it is also used for Raynauds which I acquired with ADT and still have. Its just not bad enough for me to take drugs for that reason only.

Thank you for your input. Unfortunately Jim is on lifetime ADT, so a year into treatment he is wanting some "help".

Daily Cialis protects penile tissues and helps with LUTS - the dose is 5 mg/day. "As needed" is is to help give him erections for sex. It works only if he has libido. The "as needed" dose is 20 mg.

He is actually wanting it for sex. Does the daily dose help with that at all?

So now I understand it doesnt give "libido". In Jims mind hes wanting to be intimate again very much.

I actually was worried he was producing testosterone, but nothing is functioning and he "assured" me his testosterone is still gone. He said its a "mental" thing of missing his wife.

The daily dose builds up over time and should help some. But he can do both.

Cialis is called "the weekend drug" because it has a long residency in you body. From what I read, and my experience taking the 5mg dose daily eventually builds to roughly the same dose level of the single use. I also understand that 20mg gives the maximum useful dose, so taking double doses doesn't enhance sex much and may be more effective in triggering undesired side effects. I'd suggest going with daily and taking a 20 when a sex event was likely within the next day or two. Cialis isn't instant like Viagra, so needs more time to build.

Would daily low dose generic Viagra help penile tissue just as well as the Cialis?

Our experience has been positive using viagra, and a pump to maintain the erectile tissue. Now, I have a prescription for Trimix which is freaking amazing. And to help with libido, we substitute lots of naked cuddling and caressing. This combination works extremely well for us.

Good to know. Not sure why the MO picked Cialis. He said there's other things to try if this doesn't work.

Trimix is absolutely the best . Been using it for 7 years 3 to 4 times a week.

So do guys need testosterone to "want" to have sex, or is testosterone just for functionality? I'm having a difficult time understanding the difference between testosterone, libido, ED, etc.

"So do guys need testosterone to "want" to have sex, or is testosterone just for functionality?" As for ar as I know, it's needed for everything. You won't have the interest or the ability to have erection after being on ADT for any length of time. Unfortunately, Cialis won't help.

It varies depending on the individual. Some men still very much want and need sexual intimacy, even with castration level testosterone.

That is good to know. I was concerned that being interested meant testosterone was present. He's been T <10 for well over a year.

My experience with daily Cialis was caused headaches.Changed to other option tadalafil 5mg daily and in the right "moment" can achieve partial function enough for intimate fun but not penetrative sex

They actually gave him tadalafil 10mg.He said they could adjust up or down.

Most men, I suspect, have little or no desire for sex while they are on ADT. For me, it happened within a month of the injection and lack of interest continued for many months following the last injection. A real bummer and among the many side effects of ADT.

I was in a study conducted by U of MN having to do with men with PCa and their sexual lives during treatment. They provided sildenafil 20mg tabs (3 tabs each or 60mg total) x 3 times per wk. And, also suggested "as needed." They also provided a penis pump to keep the parts in working order. With zero desire for anything sexual, it was an effort to follow through with this regime; however, I am very thankful that I did. Btw, among ADT's lovely side effects is genital shrinkage, particularly if one doesn't take ED meds and doesn't "exercise" his genitals.

Yes he's experienced that side effect and is quite disturbed as he had no idea that was among the many losses.Thank you for your input, appreciated

I just had a sexual health doctor appointment yesterday, and I'm trying the low dose daily Cialis. I'm hopeful it will supplement the Sildenefil I take as needed.

My first post was about the differences between libido, sex, and desire ( if any 😄).

I am waiting for my first ADT shot to kick in, so can't speak from "after" experience yet. But at 66, I've learned that my need for physicality as part of my love for my partner is not at all dependent on penetrative sex.

But it sure helps😁

And I sure sympathize with not having the needed info from the medical folks BEFORE side effects set in! Like being told that there may be muscle mass reduction without mentioning the penis or what to do about it 🤦.

Without this community, I wouldn't even know what questions to ask!!
