I’m getting worried should I be .

Hi guys , just need a bit of advise from you all please ok the last 2 blood tests johns psa has risen slightly it was 0.07 in March 0,09 iat beginning of may and now 0.13 in June our oncologist said she isn’t worried at the moment but is definately keeping an eye on it . He had another pmsa scan in March and it showed all lymph node involvement barring one had resolved and all Mets in spine barring 2 had resolved and the 2 that were left had shrunk by more than half . But it worries me that psa is fluctuating is he going into hormone resistant now ? Also last week when he was up and down under his car and sweating buckets as we live in dubai when he came home he had a tiny blood clot in his urine . We told the doctor yesterday and she said if it happens again you need to see the urologist it happened again today he was working onsite in a roof that wasn’t air conditioned and about 140 f and he went home for a shower and he said it happened again . But he wants to give it a couple of days ?

Please any warriors out there that can tell me if I need to be worried

Heat stroke can be life threatening. If blood clots are coming through is kidneys, they may also be in his lungs, heart, and brain.

First thing you need to do is to buy Urine Dip Sticks and test urine at least 3 times. If Leucocytes, Leukocite Esterase, Bacteria etc come positive.. then he may need antibiotics for UTI. If urinalysis come clear then see urologist for further evaluation.

A tiny rise from 0.07 to 0.09 should not be a concern. If it becomes a trend of rising PSA at least 3 or more times then we need to think about it.If he has any infection in Kidneys, bladder or prostate, it can cause inflammation which is prostatitis. And it is well established that inflammation can show rise in PSA. Even irritation of prostate by any reason can cause some increase in PSA.

Now if he is dehydrated due to working in hot weather and not drinking a lot of water, the blood (serum) can get concentrated and that concentrated blood can show increased PSA.

Dear LearnAll thank you so much , i really appreciate your reply and knowledge that all makes perfect sense . we live in such a hot country and at the minute its around 42c so you definitely need to keep hydrated i will make sure to get the the pee sticks and make sure he is drinking all the time as sometimes we forget . i really cannot thank you enough for pointing it out .have a great day x

We live in Bahrain and my husbands psa has gone from 0.03 to 0.04 to 0.07 and now 0.12 . I will follow your progress …

You're missing an important detail: what therapy is he on right now?

He is on Erleada , zoladex and xgeva . He still has his prostate .

I can't tell what the question is. I have had blood clots and the solution to that is to be catheterized, which means he wouldn't be able to work. But it's more important to clear up the blood clots.

No he had a tiny blood clot last week and a tiny one yesterday but both times it had been after a hard days work in tremendous heat around 42 c and not drinking enough water I was asking if that could of caused it . And why his psa has slightly risen and should I be worried but I’ve took advise from LearnAll and am waiting for some urine dipsticks to see if he has a uti and he is also drinking an abundance of water so we will see what happens over the weekend if it happens again we will make an appointment with the urologist .

You should always be concerned, but not overly so, of blood in the urine, or in the feces. The advice given by his doctor sounds right to me. I would doubt, although I sure don't know, that the blood clot is related to the prostate cancer.

Your husband should be very, very, very careful about doing anything strenuous in the heat, especially since he was sweating buckets. It is essential, absolutely, essential that he be completely hydrated with lots of fluids during and after this type of activity in the conditions. It can be fatal. I know. Heat stroke and heat exhaustion are times for going to the emergency room. You should look them up to get a better understanding. I have personally got almost to heat stroke working on a roof in very hot conditions. Given what you have said, it is unlikely to be related to the cancer.

I have had fluctuating PSA's. My doctor says he looks at the "doubling" rate to evaluate how serious this might be. Doubling can indicate rapid growth, which may or may not be important enough to treat. His doc should make a call on this. When I complained about a pain on the upper ilia he got me in for a bone, standard CT, and then a PSMA PET/CT scan. I had definite spread of cancer cells to multiple lymph nodes which is not good at all. But the PSA itself does give you that info at all, and the fluctuations may not be serious at all.

Personally, I find the replies about specific situations are interesting and may lead you to intensely quiz his physician, but none of this is diagnostic, at all. I wouldn't be concerned unless the doc was. That doesn't mean all docs are the best but most all are competent provided they specialize in urology with significant cancer treatment experience or they are oncologists who specialize in urologic cancer.

Thank you Maggiedrum , that’s the annoying part he does know about heatstroke we are from England but have lived in dubai for 14 years and boy once it hits may it begins to get hotter and hotter by the day . He said i think I haven’t been drinking enough so now I’m on his case constantly I call him while he is on-site to remind him to drink . But any think big or small we put down to this awful disease . I’m always worried and concerned . I think I drive him mad calling txting ringing him while we are apart . But I love him and that’s my job . I’m just glad I have this site and all you lovely people to help me through this journey , again thank you .

PSA is diluted in the body’s blood plasma and extra cellular water. About 65% of the total body weight when fully hydrated. When there is dehydration, as he no doubt experienced, the concentration of various substances when measured in the blood naturally goes up. So those changes in PSA probably reflect the dehydration and not a change in the cancer. Dangers of dehydration and heat stroke in such temperatures is grave and real. He needs to avoid working in the hot hours and drink an enormous amount of water with electrolytes, particularly sodium. Gatorade or Pedialyte or whatever is available
