Need inputs on PSA trend

My dad was diagnosed laat year with stage 4 prostate cancer and is now bedridden due to metastates to spinal cord. He is on *Zolodex injection every 3 months* for cancer. All other meds are just regular ones like hypertension, painkillers, stool softeners, vitamins etcHere are the PSA results and I wanted to get your inputs.

Does it seem like the medicine is working?

We are based out of India.

Down is the right direction. He should be getting more than just Zoladex, however. The latest advance is called "triplet therapy" which you can read about here:

He should have been offered at least Docetaxel chemotherapy as an additional treatment or better yet, Zytiga and Docetxel as TA pointed out IMO.

It looks like his PSA nadir may be around 14 and if so, he has had a relatively short duration of benefit on ADT. This combination generally indicates that chemotherapy will be more effective than hormone suppression. This is based on my limited knowledge and I'm not a doctor so if possible, I'd try to find a doctor that specializes in PCa to discuss this.

Thanks Allen and Sam for the feedback. Last time we checked with the uro oncologist they felt chemo would cause more issues like fluid retention since the patient is bed ridden. I plan to meet the doc again next week with these results to see what they suggest. My dad is also quite reluctant about chemo treatment.

I have stage 4 metastatic prostate cancer...I took lupron injection every 3 months since Jan 2021, started xtandi Jan 2022 for 4 months. Psa kept rising every month, xtandi started giving me pains in back, legs, hips...so dr stopped it & said its not working...he said next in chemo & I wanted to nit take it in the worst way. So took 1 on may 6. I was fine for the 1st 12 days, then pain & fatigue started & a week later its very bad. Had a dr prescribe ultracet...that helps but I have to take it every 5 1/2 hours or the pain comes back, very bad. So no more chemo for me & I just had another lupron shot...& taking prednisone. Will take vacation from chemo & seek alternative. Will report when I get through.

They usually prescribe 10mg of Prednisone per day for the fluid retention when you are taking Docetaxel. That works fairly well. Wishing him the best.

hey OP we are from india too.

I understand the treatment philosophy is different in India. I had hard time finding a doctor who would add second gen to lupron. Most doctors in india, unfortunately, think adt is enough for your case.

you can measure T levels to see if medicine is doing what its supposed to do.

His PSA is now down to about 8.4 and overall he is in good spirits. From a treatment standpoint it's just Zolodex now and the next one up in a couple of weeks.
