I am on Abiraterone and have both high and variable BP
Is there any data on abiraterone and ... - Advanced Prostate...
Is there any data on abiraterone and blood pressure
Fron a Phase II trial (2018):
"Grade 3 to 4 adverse events of interest included hypertension (9 patients; 15.5%)"
Yes. But the cause may be not enough prednisone. If you have low potassium and edema too, discuss increasing prednisone slightly. Eplenenone was effective in one study:
Abiraterone seemed to be the most likely cause of my significant BP increase, and here is my post from a few months ago regarding that experience and what brought my BP back down:
Prednisone dosage increase to combat Zytiga induced BP increase: Risk of hypertension in Cancer patients treated with Abiraterone: a meta-analysis - PMC

"The influence of prednisone dose on the risk of hypertension in patients taking abiraterone
Due to the heterogeneity of the above analyses, we explored its underlying causes. Abiraterone is usually given with small dose prednisone to alleviate side effects. In these clinical trials, either 5 mg or 10 mg daily was used in these prostate cancer patients respectively. We have performed subgroup analysis of hypertension risk with abiraterone among patients taking abiraterone 5 mg prednisone daily versus 5 mg prednisone twice a day. The incidence of all-grade hypertension with abiraterone was 16.5% (95% CI: 7.5–32.7%) among patients taking prednisone 5 mg twice a day, comparing to 32.4% (25.4–40.2%) among patients taking prednisone 5 mg once daily. However, the difference didn’t reach a statistic significance (P = 0.07).
Similarly, there was a difference for high-grade hypertension between 10 mg and 5 mg daily prednisone. The incidence of high-grade hypertension with abiraterone was 2.5% (95% CI: 0.7–8.6%) among patients taking prednisone 5 mg twice a day, comparing to 7.5% (1.8–26.2%) among patients taking prednisone 5 mg once daily. However, the difference didn’t reach a statistic significance (P = 0.25)."
Do you take a corticosteroid with the AA? How much? If you're taking prednisone, a first step is to increase the dose (after consulting with your MO). That helped me but did not completely eradicate the issue. 12.5-25 mg of HCTZ daily has my BP about 120/65.
When you take BP sit down for 5 minutes. Take a reading then another. The lowest is an accurate reading. Our BP varies throughout the day. ZYTIGA has high BP as a side effect. If yours is too high lose about 5kg and watch your diet. Waist measurement is the best indicator.
I had been on Abaraterone and prednisolone for about 3 months when my routine pulse monitor showed my pulse at 33bpm! BP was fine. At age 71 this would be a miracle.
Referred to cardiologist who said it could be the abaraterone causing an 'ectopic' heart beat - part of the beat is coming from the right side of heart not the normal left. I had a 24hr heart monitor trace and it showed that it was doing the irregular heartbeat about 45% of the time, when the average is 1 or 2%. I felt ok, but cardio said that over time it would weaken the heart.
My oncologist refuted that this could be the cause and after a bit of professional 'debate' I have been put on Verapamil which seems to have settled it down - 10 months on and all good.
