A nice overview from the government. For many of us not a lot new here but speaking for myself, I would have learned a lot very rapidly if I had read this 4 years ago (right after diagnosis).

Thank you, read every word and will have my husband read this tonight! Cancer has rocked our world and can’t imagine living with out my husband of 40+ years. I want to learn from all of you…. A must read for the care givers, the wives that never thought too much about prostate cancer. There is a saying in Italian “Ancoro Imparo” (I am still learning). Thank you RSH1 !Karen

You are very welcome. My wife feels the same way. For her sake, I do not want that to happen. I have a 9-year-old boy (sitting next to me right now working on school). If I only had to think of him, I'd be fine going in 12 years but, because of my wife, I need to try to go longer than that.

Some of the MOs that I have met should be required to read this and be tested on it.

It does seem like a hole to me that patients aren't at least provided links to government cancer websites so that they can understand what they might expect. That would probably meet lots of resistance from medical lobbies. Many of the patients would wind up knowing far more than some of the MOs.

Some MOs are great but like in any field, you have the good and the bad.

Excellent. That took me back about four years when I first began my own (amateurish) researching. The major change has been the doubling up of treatments on the front end.

None of the hormonal treatments directly attack already existing PCa cells? Only the means by which they grow? Do those already existing cells die before they adapt? Or an I off the main track? I've always struggled with that issue.

I think that ADT hastens the death of some of them. And it decreases the division rate. But I think that one of the reasons that RT is so effective when used in conjunction with ADT is that ADT primarily weakens the cells and makes them easier to kill.

And as you say, they adapt. That is the big issue. Change the growth environment and you force AR overexpression, adaptations, and mutations. Now you have a more difficult population to deal with.

The overview is great but leaves out advances in estrogen therapy. It is fascinating to me that it is plainly spelled out that the introduction of estrogens in high doses reduces testosterone and brings about "ADT". But the first four doctors I talked to were oblivious of this simple fact.

Hi dhccpa I also struggle with those issues and i think everyone's immune system reacts differently
