Hey guys P cancer 2. 5 years ago, removed prostate

bad margins post surgery, started lupron 2 years ago along with radiation.

(Triple Play) #9 Rad. treatments went well ,responded nicely to lupron

Dropped PSA immediately , undetectable last 1.75 years

Should I continue Lupron ?? .. Thoughts ?

If no cancerous lymph nodes or distant metastases, ADT for anything over 18 months is probably all you need.

Thanks for the reply TA. by the way I'm 58 years oldSo you think I should continue Lupron?

I have been at <.01 for most of the last 2 years.

Dr. says he would suggest giving it a break and see what happens.

Thinking my 39 radiation treatments and lupron may have done the job.

I’m in the same boat. Is cancer is gone or dormant? I get no response from doc. Been undetectable for 6 mos.

The OP isn't high risk having primary RT, he is recurrent and just had salvage RT.

We don't know what his Gleason score is and he is recurrent after RP. Wouldn't that make him high risk? If he is recurrent doesn't that mean he has metastases (although not visible on conventional imaging). I don't know. Maybe the study I cited does not directly address his situation. Thanks.

"High risk" means localized before any therapy. Kishan used the NCCN definition.

Hopefully, he only has some cancer that has grown into the prostate bed, not metastases.

It's an important study - in fact, I'm writing an article about it.

PSA dropped to.17

My Gleason score was 4+3 (7)

Cancer was in prostate bed..post surgery...... 39 radiation treatments

After 24 months of Lupron and 18 months undetectable PSA <.01

I am thinking of getting off Lupron Thoughts ??

The post-prostatectomy psa might have been due to residual prostate tissue left after surgery and not due to cancer. But, since they didn't know they treated you as if you were high risk. I think you are a safe bet to go off ADT since the NCCN guidelines say 24 months.

At 3 months, it is never due to residual prostate tissue. I assume he maent 3 months and not 3 weeks.

Thanks for the clarification. He could have been a little clearer on the timeline. Nevertheless, why can't remaining prostate tissue contribute to the presence of post-prostatectomy psa? Why would it stop at 3 months? I found it interesting to say the least, that Dr. Kwon discussed treating a man after a prostatectomy who was found to still have his prostate.

Yes. That is long enough even if pelvic lymph nodes were detected.

What was your Gleason score? This could affect what you're comfortable doing with ADT.

Thank you sir...... sometimes my silly humor gets me in trouble..... (but I believe it helps)... When I was getting fried by Dr. Zeleksy in MSKcc I was fooling around and laughing with the technicians. Another paYtient also getting radiated approached me and said "You're the happiest guy I ever saw who's got cancer"..... my response was "I rather laugh than cry".......
