I would recommend Lu177. According to this study thelancet.com/journals/lanc... : "[177Lu]Lu-PSMA-617 compared with cabazitaxel in men with metastatic castration-resistant prostate cancer led to a higher PSA response and fewer grade 3 or 4 adverse events. "
Usually these decisions are tough. These guys seem to have made your decision easy.

I'll be in the same boat at some point down the road and have already discussed this with my doctor. He feels I should do LU-177 first, but said he felt that he couldn't say it's a better treatment at this time. I think the side effects of LU-177 are better than either Docetaxel or Cabazitaxel.
But you also need make sure you are PSMA avid with a PSMA pet scan. There are around 20-30% of patients who are not PSMA avid.

Gregg, do they or plan on offering LU-177 at a VA facility? Thanks I'm in the same basket as you. (25 months on Zytiga), planning my next move.

Here in Northern California, my doctor said they are gearing up for the treatment. They already have a PSMA PET scanner in at the Palo Alto VA and will soon have one in Sacramento. My doctor said the main thing is getting the tracer since the scanner is just a PET scanner with different software.
He expects to be able to start treating with LU-177 around 3-4 months after FDA approval since it will take some time to get everything set up.
That's what I know at this time.
He also said that at this point moving forward, the PEACE 1 protocol (ADT + Docetaxel + Zytiga) would be the best first treatment at stage 4 diagnosis followed by LU-177 when that becomes ineffective. That's what he thinks now.

I had my 3 rd infusion of LU 177 since Sept. I had also done my Taxotere in 2019. The side effects of LU 177 is a lot more manageable than Taxotere. Hope this is helpful
