Why would psa come up and Alk phos come down. This is all so confusing. I’m so sorry to be redundant just trying to understand. I’m not getting answers from our local Oncologist and have to wait for our other Oncologist Dr Beltran to get some answers. Sorry

The PSA can go up from the dead cancer cells. The Alkaline Phosphatase going down is a good sign.

But more importantly, imaging is what is going to tell you whether the treatment is working and you make your decisions based on that. I would not try to read too much into the PSA number. It's not as important. Measurable is not always Meaningful.

Alkaline phosphatase is an enzyme which is secreted at a normal level when normal wear and tear is happening in bones. So the normal range is 45 to 117 u/L.When the process of repair of bones are going at higher level, the level of ALP rises...Once the repair is done the level of ALP starts falling and reach at normal range. So falling ALP is usually a good sign because the repair process is not needed as much as not much corrosion is happening to bone anymore. ALP comes from 3 sources in normally healthy man..45% of ALP comes from Bones (Bone ALP), 45% ALP comes from Liver (Liver ALP) and the remaing 10% comes from all other tissues such as intestines etc.

Now, Coming to PSA,,,,,Prostate cancer cells are filled with PSA and when the treatment pops them. .suddenly a lot of PSA is released in the blood and that shows as high PSA. After 4 to 6 weeks, PSA starts falling and comes way down if treatment is working.

If both PSA and ALP are rising, simultaneously it not good sign.
