Lu 177 along with AC 225

We are looking at treatment at Heidelberg Hospital for LU 177 PSMA. I noticed they did not offer AC 225 along with LU 177. I believe other hospitals do offer both treatments together. If you have received LU 177 PSMA along with the AC 225 in a different hospital in Germany would you please let us know. Thank you

They do offer it. Most patients get Lu177 and do not require the combination with Ac225. But you can ask for it, if you want the combination.

Heidelberg is where I went. Did you contact the international office at Heidelberg?

I contacted the international office which is on the University of Heidelberg web site. They very promptly replied and uploaded my scans. They were reviewed and I was rapidly given an appointment. Saw Dr. Merkel when I was there. Was a very kind of mechanical process but I did not have problems with it. I spent 2.5 days in the hospital, received the treatment on day one, and fluids for a couple days. Scanned (scintigraphy) the next day to confirm that uptake corresponded with sites defined on the PSMA scan. Total cost US was $12,000. heidelberg-university-hospi...

Not with PSMA scan. I had gotten scanned in Pet Imaging Center of South Florida when they first got Pylarify as I had gotten on their list. Uploaded data to Heidelberg. I received combination of 177 Lutetium and 225 Actinium. Was told one dose and follow psa which to date had remained low. Only scan was the routing nuclear scintigraphy scan they do after lutetium to see if isotope uptake area correlates with PSMA Scan. I did get a dry mouth after the Actinium it is slowly improving but I saw my dentist and have dental care plan in place.

I had 2 treatments of LU177 at University Marburg Germany Hospital. I inquired about the LU177 & AC225 cocktail before my 1st treatment and they recommended against it due to quality of life. They said I would probably lose my saliva gland functions because I have high PSMA expression in them. After my 2nd treatment of LU177 with mixed results (PSA drops over 200 points then comes roaring back) they are moving me to AC225 on my next treatment in a couple of weeks. I again inquired about the cocktail and they didn’t see a need to add LU177 after the results of the previous 2 treatments. Their reasoning was I have high PSMA expression and I absorb the radiation with high intensity in my Mets so the Alpha particles of AC225 provide higher linear energy transfer to the cells. This can produce substantially more lethal double strand DNA breaks per alpha track than LU177 beta particles when traversing a cell nucleus. Sounds logical to me.

Ac225 is working more effectively against the tumor, especially bone mets. But as you wrote "They said I would probably lose my saliva gland functions ..." that is still correct. Therefore doctors use the Ac225/Lu177 mix to reduce damage to the salivary glands. It is not the same mix at all hospitals. Some just add 20% Ac225, others 60%. The 60% damage the salivary glands more than lower percentages of Ac225.

I try to reduce side effects as much as I can. Therefore I would take a mix and discuss the percentage of Ac225 with the doctor. Destroyed salivary glands are a pita.
