Just got the latest blood results - which show a further rise of PSA - thus confirming I have reached Castrate Resistant. PSA was falling nicely until a nadir of 1.5 at end april, and suddenly changed to a rise with a PSADT of about 200 days. This surprised me since I had expected a flat nadir followed by a gradual rise as the cancer became castrate resistant, and not a sudden reversal to exponential growth. I have therefore started on Xtandi - done 2 days so far without side effects (how long before they kick in??). Next PSA scheduled for 4 Nov so that will be interesting. Details are on the plot - note the log scale and the last point which is a projection to guage Xtandi effect when it comes.
My oncologist reckons that the high PSA at the start means that my cancer is highly productive of PSA and therefore will show a good sensitivity to changes - none of this 0.0x PSA changes for me!!)
The 21 months to castrate resistance seems par for the course as far as I have read so expectations for future developments should also follow the trend - down the well trodden path that so many of you have traveled before me. I am still symptom free, enjoying life, and making the most of each day. The future will take care of itself.
