Does anyone know if orgovyx can be stopped and Firmagon be restarted seamlessly?

My husband’s doctor has suggested he switch from Firmagon to orgovyx. He was due for a Firmagon dose yesterday. PSA is at 0.12. We are not sure what the copay will be with his WellCare Part D. Originally we signed him up w Aetna Part D and it was transitioned into WellCare. My husband takes no meds so the difference in formularies by carrier was not on our radar, but Aetna was apparently better than WellCare, so I’m anticipating no coverage. If that means a $600 monthly copay he may stay w Firmagon. But we don’t know.

We have the first free month of Orgovyx in hand and he could start today, use it for the two free months, then decide when we know his coverage, but I don’t know if switching back is seamless.

Surprisingly we have not been well advised by his doctors on this. But that’s another story.

Any guidance will be appreciated.

Thank you

I was on Relugolix for one year, and when the trial ended I immediately switched to Firmagon. No problems, and PSA remained undetectable.

Thank you! That’s good to know! He has been on Firmagon for the second time for about a year and this time it never took him to undetectable.

His first go round with Firmagon he reached undetectable pretty quickly and T stayed down around 10. This time T has bottomed out at about 25, but it took a while to get there. We are hoping the Orgovyx will drop it that last little bit (PSA and T).

May I ask why you chose to go back to Firmagon rather than stay with Relugolix?

I was in the phase 2 clinical trial of Relugolix, and when the trial ended I had to switch.

Question for you. Since Firmagon and Orgovyx are the same biochemical mechanism....why would you expect a difference in result? I was on Firmagon...and now 2 months on Orgovyx...and the result is exactly the same. Undetectable PSA and T = 17. Wish you and your husband well on your journey.

I asked these questions of Tim, I'm curious if you can answer any of these questions as well- Did your stool change after being on Orgovyx? Mine is like hard nuggets now. I've never had that before. I'm also having heart palpitations. Not sure if its from Orgovyx or from covid vaccine. I started Orgovyx 22 Feb and 2nd covid vaccine shot on 26 March. Two days after that, the nerve problems in my shoulders and arms began, as well as the heart palpitations. Did your stool or heart change in anyway between the orgovyx and firmagon? I may have to switch off the orgovyx to see if my palpitations stop.

I also am having heart palpitations from Orgovix. They started the day of my first pill. I took pills for three days. Did not take one today to see if it makes any diff in the palps.

I had none of those symptoms on Orgovyx or Firmagon. But my hot flashes were more frequent with Firmagon. Both drugs kept my T under 15 and PSA <0.1.

Hmm. I think the heart abs nerves are from Covid vaccine but i think the stool might be from the orgovyx.

I had none of those side effects on 3 months of Orgovyx....just hot flashes and some lethargy (less than Firmagon). T at 17...undetectable PSA on both.

Hello, George, I checked w my husband. He said he’s had none of the symptoms you describe from the orgovyx. He’s also had both doses of the Moderna vaccine. He said he feels a little better energy level-wise on the orgovyx than the Firmagon, but he’s only been on the orgovyx for one month. Blood work will be done on Friday and I’ll update w his T and PSA readings.

This is what I just wrote to JRP - I am well hydrated. I've been doing some online research. Three new issues occurring all at the same time _ heart palp/PVC, muscle twitches on my left arm and left leg, and dry nugget type stools. This all started after the 2nd covid shot ( I had Levaquin damage a year ago so I may be more susceptible). One of the causes of muscle twitching is neuropathy, which is what I started having two days after the covid shot. Because this causes an electrical imbalance and the twitching, it can send signals to my heart as well, and possibly cause the PVCs/palpitations. I don't know if that would cause the stool changes. I assumed it was from orgovyx but it didn't start until five weeks after I started orgovyx (one week after the covid vaccine.) I'm going to start taking my benfotiamine again and maybe that will help. Gonna try a massage and keep exercising and phys therapy to get blood blow into the two shoulders that are still hurting a lot.

Are you keeping yourself well hydrated? Any changes in diet? No sign of any GI effects.
