New study below [1].

The 'Prognostic Nutritional Index' [PNI] is new to me, but here's the definition:

"PNI was calculated as 10 * serum albumin (g/dL) + 0.005 * total lymphocyte count (per mm3)."

Not surprisingly, more is better:

"Patients with elevated PNI had better clinical outcomes."

"Elevated pretreatment PNI was a favorable prognostic indicator for PCa patients treated with ADT."

Albumin & lymphocyte count are inflammation markers. Here are two old posts: [2] [3].

Patrick...the studies usually talk about "PRETREATMENT" albumin etc. How do you interpret if some one had High PNI before starting ADT and then, PNI fell down only to recover fully after a few months.

I do monitor NLR, PLR and PNI each month. Recently, I read that derived NLR is more accurate than plain NLR.

derived NLR is calculated using Leukocytes minus neurophils and then dividing by abs lymphocytes.. These ratios are fascinating and are really useful tools to know cancer

activity and inflammation in the body.

I fear you got the dNLR wrong LearnAll.

The "derived" has to do with the lymphocytes that are entered by (WBC-ANC) => White Blood Cells minus Absolute Neutrophil Count.

dNLR =ANC/(WBC-ANC)

dNLR is always less than NLR as its denominator, apart from the lymphocytes, comprises everything else, like the Monocytes, Eosinophils, etc.

I have compiled a spreadsheet of my blood tests since 1988.

During my years of "innocence", aka pre-PCa, my dNLR fluctuated between 1.33 - 2.33.

Three months before RP it assumed the highest recorded value of 2.8.

Post RP, it moved bellow 2.0 with a tendency to decrease with time.

Thank you justfor for the correction.

I have been tracking PNI. It was good before ADT and marginal and declining with ADT. Cheers, Phil

Hi Philip Could you define numerically what you consider to be good, marginal and declining values of PNI? Thanks Jurgen

Hello hansjd, I obtained the limits from the following article: “Prognostic value of selected preoperative inflammation-based scores in patients with high-risk localized prostate cancer who underwent radical prostatectomy”, PMID 30122947. Please search Pubmed for additional papers. Cheers, Phil
