Met in c5 c6 :-(

Now on Olaparib off Xtandi ...psa rose to 4.8. But, met in C5 C6 area is really painful. Pain in my neck, left shoulder, arm and fingers are tingly. Very limited motion now in my left arm. CT scan showed narrowing ... thanks to my wonderful PCa onologist, I got into a neurosurgeon right away. He thinks it is something called Parsonage-Turner syndrome. Has me doubled up on Gabapentin and on a steroid blast and the intention of starting physical therapy in January. Not helping .... I was radiated to the max back in 2012, so that option is no longer on the table. Not sure where to go from here, but I do need to get some pain relief so I can sleep. I have some left over Oxy ... but that doesn't do anything either. So much for a "Merry Christmas" :-(

By your description of pain..to me it appears as Nerve Compression Neuropathic Pain. Opioids like Oxycontin do not help much as pain is coming from nerve compression.

The nerve compression needs to be released...either by physical therapy(if possible) other wise surgery. Meds which can help with neuropathic pain are Gabapentin and also steroids (by suppressing inflammation at the site) You are already getting these two. That's Good. Lets see what physical therapy does. Good Luck.

Unfortunately, PT appointment isn't until January 7th ... and that is just to evaluate and start to schedule.

In the meantime, ask your doctor if he can give epidural prednisone in and around C5-C6 to kill inflammation and provide temporary relief. Ask if can go further up on dosage of Gabapentin. Its a safe medicine and can be used up to 3000-4000 mg a day.

What dose did you get to C5/C6 last time? They can usually give a second dose for pain palliation. Xofigo is great for that too.

The radiation oncologist looked at what I had back in 2012 and said no .... that she wouldn't have given me as much as I got back then and would not do any additional radiation. If the pain doesn't get under control, then it will be surgery :-( But, if that helps, I'm for it. Something has to give.

Do you have any side effects from Olaparib? Dose?

You say you are off Xtandi, I assume it stopped working. Saw something about Xtandi and PARP inhibitor acting in synergy. Something about Xtandi tendency towards forming BRACAness , where taking PARP inhibitor would work towards repairing resulting somatic germline? DNA damage

I haven't been on Olaparib long enough to notice any side effects (4th day today). Yes, Xtandi did stop working. I'm taking 300 mg Olaparib morning and night ... so total is 600 per day.

Hey Alvin735!

I had a lumbar injury pushing vertebrae 4 & 5 in to the extent that they began to fuse to each other and the vertebrae next to them. You have my empathy. I had severe sciatica.

Confer with your health care professionals about these suggestions. You don't need to find yourself in a worse situation by following what I think may be beneficial.

Have you tried a chiropractor? That may help. Many today use an activator. You can use your search bar to look that up if you don't already know. I wouldn't think manual manipulation would be safe. Manual manipulation can be aggressive. I question the safety of it if I were in your situation. Not my call. I have had quite a few adjustments with the activator. It is very gentle and can be used even on the elderly because the force can be adjusted.

Maybe something as simple as a Teeter Hang Up to allow the weight of your head to give you a bit of decompression time on those vertebrae in your neck. Be certain you can use a Hang Up and it would be best if there was someone with you. I still use gravity boots.

Sorry, Alvin. I'm guessing you have tried, or thought of trying, Radium 233? My onco says it is the next step for me, with ADT starting to weaken.
