Dad is now ADT resistant. Can I see a hope for his survival. Seeking best treatment in India. Advice please.

ADT resistant does not mean treatment resistance. Its only treatment resistance to one type of treatment and that is androgen blockade,
If you provide details about your dad's situation, some one can show you the direction you need to go. This forum is full of some very smart and knowledgeable people.
What ADT drugs has he been on? There are many others and each could provide years of quality life.
He is on zoladex only for almost 3 years. He had a solitary femur met. Was given sbrt and ADT.now his psa is rising rapidly with low testosterone. Last was . 06 which has increased from <.008 in almost 5 months. Worried about prognosis. Please suggest next steps
In the US, approved drugs for metastatic castration-resistant PC include docetaxel, abiraterone, enzalutamide, radium 223, sipuleucel, and cabazitaxel. In India, you can also get Lu-177-PSMA. If he only had the one met, radiation to his prostate may be helpful too.
What is the life expectancy of Adt resistant CA prostate. Also when shoul we start the next treatment and scans
There is no certain way to tell who will live how long..people with CRPC have lived anywhere from 5 to 15 years even....because we have so many new treatments
like Tall Allen said above...Abiraterone. Docetaxel, Enzalutamide, Radium223, Lu177.
And I know most of new treatments are available in India. including immunotherapy.
Just want to point out that his PSA is .06. This is not considered castrate resistant by any standard I know of. He may continue for quite a while only requiring primary ADT. When his PSA gets above 2, then his doctors can begin to consider whether he is becoming castrate resistant. And of course as discussed, he has quite a few options at that point. PSA increases of tenths or hundreths are meaningless to metastatic prostate cancer patients.

Did you hear what Gregg just said...."your Dad is not ADT resistant yet". Touch Dad's feet, get his blessings and give this good news to him and entire family.
Like Gregg said as long as his PSA stays below 2 ,he will be considered Castration sensitive.
However, abiraterone still can be added to optimize outcome further. Put good amount of turmeric (haldi) , Ginger (adrak) and Garlic in his lentil soup(daal) and curry(Sabzi). Good luck to you and your Dad.

Curry is a British word....for Hindi word "Sabzi" Sabzi is a general term for any vegetable which is cooked in Indian way. Lets take an example: Take a cauliflower, cut it into small pieces. Then, for the paste (masala) cut onions, garlic, red tomatoes , ginger..into very small pieces. Add salt per your liking. Add grounded turmeric , coriander powder,
Put one or two teaspoons of oil of your choice . Let the oil get heated. Then, throw some cumin seeds into oil. Once you see cumin seed turn brown, put all your paste (see above into the pot, stir it and pour a cup of water. Let the paste simmer in light heat until it becomes uniform and mixed looking like paste. Then cauliflower pieces are added and let this simmer in light heat until this stuff gets cooked well , In between, keep stirring every 2 minutes. Once ready, sabzi can be eaten along with bread. This is very basic way of making any veg curry. In place of cauliflower, Cabbage, broccoli, okra, potatoes any veg. can be used BUT the paste part remains the same. Sabzi is anti cancer food.

The Panick of not knowing is real . Fear of losing a father hard... For us that cloud of reoccurrence hanging overhead us never really stops .... Zen mode Is needed... to skip along without obsessing about pc.

I learned one of the most important lessons in my doctor's office. I went through the treatments available with my doctor and after she told me about each one, I said "and what happens after that?" eventually we got to end of the list and after my final "and what happens after that?" she said "We'll have to get comfort care for you." I immediately broke down crying and thinking to myself "Oh my God, I'm going to end up in hospice!"
After I calmed down and left the appointment, I remember thinking to myself "What the hell did you just do?" I really felt stupid and never allowed myself do anything like that again.
IMO, we must stay disciplined about not living in the future, not trying to predict what terrible things are going to happen to us and not senslessly worry about things like our PSA. The time we have left here is too precious to waste, our quality of life too valuable to squander. Guard your quality of life and resist the temptation to wander into a future you can't predict.

Agree. The fear is tangible, but rising it above it and living fully inspite of it is an amazing and powerful experience. Fear is an opportunity to have courage.

It’s easy for me to not have fear when I have no signs of pc right now . If or when APC comes to eat me up again . I’ll be crying like a baby then .. I feel for those in fear . It s the super bad bad of this deal . I’m not really brave just lucky I guess .. 🌵

Yes, it's easier when things are going well. When things change for the worse there is always a relapse into grief and fear. For me, it takes me back to the trauma of that time in the doctor's office when I was given "the news". It's a struggle, but so far I've been able get back on my feet again.
No question about it, this whole thing is difficult. Trying to make the best of it is a real challenge, but there are rewards there too.

That's me too! It's easy to be brave when undetectable and feeling good. A bit of fatigue and some new ache and the facade starts to crack.

Last visit with my MO I was panicking about a lump on the side of my upper neck thinking it might be a cancerous lymph node. My doctor felt it and said, "That's not a lymph node, feels like a zit to me." I was almost surprised "What? I thought it would have to be some kind of tumor. You can actually have a zit with stage 4 Prostate Cancer?" I've become so "trigger happy" for lack of a better expression. Normal aches or pains can send me into depression or make me emotional. It's something we all have to live with. Not easy.

Yep, and it's so unfair that we still have to deal with the common cold, acne, and other such routine ailments. That said, I do believe the prednisone has helped my exercise induced asthma.

With The muscle and joint pain while being castrated if felt it could be Metz . Thank god for me it’s wasn’t.. If I had a lump on my neck I’d be a little freaky too .. I’m not really brave ,just lucky for some reason? Enjoy your Christmas !🎄😂

In my opinion
0.08 PSA is so low that it won't mean castrate resistant.
Mine moves around 0.04 to 0.08 up and down. Maybe won't go lower because I have a prostate (well at least half).
If your father has a prostate, any tiny inflamation will rise a bit his PSA.
I wouldn't panic.
Monitor that PSA. And do a Pet/TC if it rises. FDG & PSMA. But I doubt you can find any progression in images below 0.10.

He is 65. He has had RP. Then was kept on active surveillance. Had mets to femur. Received sbrt and zoladex for three years now. Psa now on a rising trend since 5 months. Gleason was 9. Latest psa of .06

Any rise in Psa grabs us by the short hairs . With this slight increase there is time to try something new ... Enjoy time with him . Deal with this after New Years. Please let me know what he’s given to bring it back down ? No gloom or doom he can deal with this . 🎄🎄
Is there any role of genetic mapping now. Blood/block or both? Will I be able to know n treat prostate cancer better?
