I haven't posted for a while but have continued to follow all. Had a PSMA PET/CT on Monday 13th May at Townsville in North Queensland. Finally have the Results. I could try to upload the whole story but that would confuse the crap out of me. Bit of History: age 62 PSA 180 Gleason score 4+3 =7 in 20/24 core biopsies in all 6 zones? metastatic disease. Staging. Radioligist conclusion : Large and intensely PSMA avid primary prostatic malignancy involving both sides of the prostate gland. There are small but focally PSMA avid bilateral external iliac nodes highly suggestive of nodal metastases. No evidence of osseous metastatic disease.
Had a 22.5mg shot of Eligard on Wednesday 05/06/19 and get another in 3 months. My question, What is iliac nodes and what is osseous metastatic disease?

"bilateral external iliac" - lymphnodes are usually near arteries. So these are next to the external iliac artery. Means they are in the pelvis, a better prognosis than outside it.
You could have surgery with extended lymph node dissection to treat this.
You sound like a good candidate for brachy boost therapy, which has a better success record than surgery or radiation. You have to talk to a brachytherapy specialist to get it. Bilateral external iliac lymph nodes are those around the external iliac arteries of the pelvis - but you should treat all the pelvic lymph nodes up to the top of where the "common iliac artery" splits (bifurcates)- lymph is a slow-moving fluid that brings cancer cells wherever it goes - many of the tumors are too small to be picked up on a PSMA scan. Negative for osseous (bone) metastases is a good thing because the only way cancer cells travel to bone is in the blood - that would mean the cancer is everywhere (systemic).
TA thank you for a very informative answer. I think the onc mentioned brachy boost therapy in December if my PSA drops below 1. I've just had another psa test so i'm keen to see if it had gone up from 180. I'll post when I get the results. Thanks again.
