My husband takes his Xtandi late at night because it can cause him nausea, he’d rather sleep through. In the past month he’s fallen asleep twice forgetting to take it which then taking it in the morning throws his whole routine off for a few days. I’m wondering how much this inconsistency in timing could effect its efficacy?

Probably not at all. Xtandi takes 28 days to reach equilibrium in your body, so a few hours off should make no difference. I now take 80 mg daily in afternoon and 120 mg on Sundays. Ends up being 86 mg average dose. 160 mg daily made me an invalid, 120 mg and I could function, 100 mg and I was improving, so kept cutting back till I was at maximum QOL dose. Still holding PSA in 0,140 range, Good luck with your treatments and don't worry too much about timing on long half life meds. likes Xtandi.
Don't know, not a doctor. Guessing once in a while not much of an issue. Take mine at 8pm. Have alarm set on cell phone for 8pm. It is very loud and very annoying. Take my 4 pills and set bottle on bottom shelf to let myself know I took them. Get drowsy about an hour and a half later so may lose the thread of the plot if watching tv. Some time after 10 pm or so I wander to bed. (This is winter schedule). I am missing one tablet. So, I took one too many, I lost a $100 tablet, or pharmacy short changed me. Don't know. I am into I don't careness any more. So I don't care. I may have missed a whole day once. Missing even a day is probably not a huge issue. But again, not a doctor.
Shooter1 is right. Missing a few hours will make no noticeable difference to the level of Xtandi in your husbands blood system. I have missed a couple of evening sets of pills, and just pop them in when I realize it the first thing when I wake up. Don't worry, but don't skip if you can help it.
