What is the latest research on Casodex? My MO in Chicago is tells me Casodex has lost favor with many US MO's and most are turning to other drugs. Presently I am on a Casodex vacation (7 months) and trying to determine if I should start Casodex again or try something else. Before the vacation I had been on Casodex for about 18 months. (50mgs twice a week). My PSA has been undetectable for over three years. Presently my PSA is .111 and taking Avodart three time a week.

Bicalutamide [Casodex] was FDA approved 23 years ago & has been available as a generic for 9 years.

Enzalutamide [Xtandi], another antiandrogen, was FDA approved 6 years ago.

"Enzalutamide significantly prolonged median progression-free survival vs bicalutamide in chemotherapy naïve men with metastatic castration resistant prostate cancer in the TERRAIN (Enzalutamide versus Bicalutamide in Castrate Men with Metastatic Prostate Cancer) trial." [1]

"Enzalutamide, a potent oral androgen receptor inhibitor, improves survival in men with metastatic castration-resistant prostate cancer (CRPC) before and after chemotherapy." [2] The STRIVE Trial.

Are you metastatic or just recurrent? If you are recurrent and Casodex is working so well for you, and the side effects are low, why change? The worst side effect is gynecomastia, which you can prevent with 10 mg tamoxifen.

I have a slight recurrence bounce. Will see what happens over the next couple of months.

I agree. If casodex still works why change? It’s cheap and effective!

When I went to md Anderson two years ago the first thing she did was take me off casodex , told me it increases psa, the numbers still went up.started on chemo when I got home

If Lupron fails I would not recommend to try Bicalutamide instead.

However, if you are hormone sensitive and you do not know if you should just observe or start with ADT, Bicalutamide can keep the PSA value low with fewer side effects than Lupron.

Here I am in agreement with T_A, stay with Casodex. Each of the ADT drugs provides a different approach to cancer. So stick with one before changing.
