Has anyone here completed Lu-177? Can you give me some insight into side effects? How effective was it? Is it worth a shot for my dad?

I had one round of Lu 177 PSMA in Munich 2 years ago and I did not have any complication. It was very effective all the lymph node metastases in the pelvis and abdomen became PSMA negative after 1 treatment according to a Ga 68 PSMA PET study done 5 weeks after the treatment.

I continue in ADT alone, the PSA is 0.08.

Around 9 thousand euros. Includes everything, Drs., blood tests , post body scan and spect study and 3 nights in the nuclear medicine ward. If a GA 68 PSMA PET is needed it will add around 3 thousand euros

Diagnosed in Nov 2017, PSA 64, Gleason 10, stage 4 with a superscan (too many mets to count). Had instant success with Zytiga, Lupron and prednisone, brought PSA down to .2, but started climbing rapidly after just 7 months. Switched to Xtandi but didn’t work at all made him very very sick and PSA climbed rapidly to 20. Doubling every 3wks. Just started taxotere with prednisone. Finished first round with minimal side effects. Doing much better. Appetite is back and gaining some weight back. Fatigue is much better. Just looking to the future. Wanting to have a plan in place for my dad. I still feel like he has a lot of life to live.

It can destroy your salivary glands, which is a lot more dangerous than it sounds.

And it can damage your kidneys, which is why they limit the number of treatments you can have.

The Lu177 PSMA has the potential to cause salivary problems and renal problems. Most of the patients do not experience problems with the salivary glands. They ask you to apply cold packs for 10 hours during and after the infusion. I did not suffered any problem with these glands. There are serious problems with the salivary glands with the Actinium 225.

The renal toxicity is controlled by the number of treatments and it can be also controlled more precisely if they do dosimetry and measure the radiation received by the kidneys.

I did not have any problem with the kidney function after the treatment. The articles I have read about Lu 177 PSMA do not report renal problems as a major complication.

I just completed mine with mixed results. The Doc called the out come stable disease, but I think he was being generous. PSA around 400 and I'm rising.

1st step is to obtain PSMA Scan. I got mine at UCLA-$2700. There are open trials for Lu-177 inHouston,Omaha and UCLA. I went to Heidelberg for treatment a month ago at suggestion of my urologist at Mayo. My disease burden is relatively low as I have had 3 radiation treatments, a Cryo treatment following C-11 choline scans and the general supply of pills and injections- everything but chemo! Heidelberg recommendation was one cycle of Lu-177 plus Yttitium. No side effects thus far and to soon to see signicant impact on PSA.
