My dad is 71 yo, stage IV, mets to sacrum, one rib and shoulder. He’s done basically every approved treatment. Showed good progress on Xtandi for 9 months then just one good month on Xtandi.
If we were able to get Daralutamide or Proxilutamide, would we see any results?

Neither of those are available outside of clinical trials, and we don't yet have proof that either is better than what he has tried. Unless he has failed Jevtana, he can try it again and rechallenge with Xtandi afterwards. Estrogen patches may still work, if he hasn't tried that yet.
Has he tried Zytiga? I have been on Lupron since diagnosis in 2015 at age 53 (stage 4, Gleason 8). PSA went from 227 to undetectable in 3-months. Remained undetectable for a year and then slowly crept up to 1.8. In December 2017, I started Zytiga + Prednisone and within a month back down to undetectable. Working well and few side effects.

Hi, My husband Sam is also stage IV, we just recently qualified finically for a drug called cometria. It's a targeted therapy drug. I'm praying and fingers crossed. Sam started his journey at age 65, he is now 74 young...
I did read an articial about proxelutamide and one called baroultamide. The head line read: Death from cancer might be over!!! Wouldn't that be wonderful!!! I pray for that everyday!!!! Please let me and the rest if you are able to get the drug. Do you know if the drug proxelutamide is a targeted therapy drug???
Check out LU177 which is in clinical trial at UCLA and Excel in Houston. Also might benefit from spot radiation (preferable proton) to get rid of the known liaisons.
I would contact the National Institutes of Health/National Cancer Institute. that is the federal government medical research facility. within the NCI page is a search engine where you put in your age, disease and zip code and it gives you a list of clinical studies and trials at NIH and at NIH approved facilities within 100 miles of where you live.
