Chris, I'm going to assume the answer is yes based upon me having something similar in my ribs that is confirmed PCa related. For minor aches, I go unmedicated. If the pain goes up, I start with Aleve. It's often effective. When it's not, I move up to tramadol. When that also wasn't helpful back in 2015, I had palliative radiation.
my husband is 12 years into a G10 with rib mets at diagnosis. four years ago he had palliative radiation to his shoulder which barely helped at all. his shoulder now points to 7 o'clock instead of 9 and is unusable. a year ago his neck started to crack so loudly that i could hear it in the other room on a quiet afternoon. an MRI revealed what his doctor called ''moth eaten''. he requires 45 mgs of oxycodone to even be able to lie back on a pile of fluffy pillows, now.

Have a look at Lutetium -177 therapy. It completely cleared my brother's 30 mets after just two treatments in Sydney, Australia. Almost no side affects.

UNFORTUNATELY, and I say that with caps intended, my husband suffers from two comorbidities, all three cancers suspected of being caused by exposure to Agent Orange. I say that with a degree of certainty as no one in his family has ever suffered from cancer and they all lived well into their early 100s. No clinical trial will admit people with a comorbidity unless is it treated basal cell carcinoma. They wouldn't even allow him into a final trial for enzalutamide after his zytiga failed. They made him wait until it became FDA approved as they watched his PSA soar into the 1000s.

I had a pretty severe version of the pain you describe, but from a one inch metatesis on C2 vertebrae much higher up. My PSA at the time was .6, so I was not expecting the tumor. An X-ray did not show the tumor. It took an MRI to locate it. Prednisone and Advil were the best pain relievers for me, better than oxycodone. 12 sessions of spot radiation finally cured the pain completely, four years ago.
Then I had my first F18 sodium PET/CT bone scan and found I had more metateses up and down my spine. If you have one, you may have more.
Xofigo reversed most of these metasteses and kept pain away completely.
Have you had a recent PET/CT bone scan? Hope you find relief quickly.

I have recently developed an occasional crack in my neck. I thought it was from the xgeva shots I was getting but my doctor said I was probably old age. I don't have any pain with mine but it is annoying.
Chris, Yes it can. I would suggest that you reach out to your doctor and advise as spinal mets can cause significant damage that has the potential to paralyze.
