What signs are the big signs in your opinion? Of dosage too low? I am nervous to ask my psychiatrist to go above my current dose (20 mg adderall IR 2x daily) because he already said 40 is a very big dose which may be true, but I'm only ever on 20 mg at a time if you think about it! I am a 28 year old female.

Okkal, it can't hurt to talk to your doctor about it! I am just begining my journey with getting support for my ADHD but tell your doctor what you're feeling. I know that due to lots of discrimination within the medical community you may still not be listened to - and that would stink - but it can't hurt to try, right?
You might have to find a new doctor. 40mg as a "big dose" is very old school thinking based on studies of childrens' doses. 60mg is the FDA limit (40mg recommended for ADHD, 60mg recommended for narcolepsy). I've been on 90mg and have heard many accounts of people having been prescribed more. I would say look up tolerance reducers such as memantine and magnesium and try it out to see if that helps. If you feel that the dosage is too low, that's all the signs that you need. Bring it up with your doc while mentioning your best efforts to maximize the effects of the meds naturally. If he/she is still unwilling to up the dose, I would have to recommend finding a new doctor. So many doctors have arbitrary limits and haven't kept up with the latest studies such as generic tests and pharmacological factors. 40mg is a moderate dose. Good luck and stay safe!!
