Besdides the baduc demograghic information... i think every great PLAN should have the name and title of oeople in attendance so that you would know who providing what services for your child. It present level of academic and functuinal performance. Measurable and anual goals. Esrended school year determination (summer school). Stste and district assessments . Be sure to work with the group of specialists and educators as you are a teacher as well, you know your child's habits, strengths and interests and personal attributes and should be mentioned in the PLAN. THAT tells me they're getting to know your child on a somewhat personal level.

The most important thing that should be listed is A Service Delivery Ststement..... such as 60 minutes per month of direct SPL outside if the general ed class. 20 minutes if direct instruction in literacy within the general ed. class. 90 minutes if indirect OT services. and 120 minutes if mental health services.

My Gson is receiving services within the least restrictive environment inside a general education classroom. I can't tell you what the OT looks like directly, but it includes a weighted vest, away from distractions near teacher, sensory breaks, extended or almost absent homework time, a visual schedule and a visual timer. There is a bit more but you get the point.
