I had:
PSA
30, Gleason 10, PCa Stage 4 localized to pelvic region. Treated with IMRT radiation + Eligard + Abiraterone.
PSA
undetectable. Stopped Abiraterone due liver that hates it (liver is ok). Will try Nubeqa, and may switch to Orgovyx after Eligard runs out in December.

ipp and salvage radiation
Rp on july of 22 for gleason 3+4 with
psa
of 16. PT3b, pN1 with negative margins and 2 of 11 lymph nodes involved. I took a nomogram online and came out 75% likely for recurrence. My first
psa
post op was .02, then .03, .04, .04, .05 in late august- so I've crossed line #1.

of 16. PT3b, pN1 with negative margins and 2 of 11 lymph nodes involved. I took a nomogram online and came out 75% likely for recurrence. My first

rising, now ~3.5. Biopsy 5 out of 7 cores Grade 2 Gleason 3+4, with perineural invasion. In preliminary post biopsy conversations the urologist has suggested either prostatectomy or radiation therapy.

Unexpected early end to ADT
Orgovyx has kept my
PSA
at <.05 UNDETECTABLE for all of the 14 months, with mostly tolerable side effects. Muscle loss, fatigue, brain fog and hot flashes. At this point with only a four month window, I am not willing to move over to Lupron or the like for fear of it causing more harm than good.

Can we trust AI analysis?
The first chose no ADT with radiation and the second chose 12 months of ADT after surgery and follow up radiation after

rise. I guess only time and luck will tell if that was the correct decision for each of us.

remains at 1.8 even though the scan showed a small tissue met to shoulder area (no bone). The met did not cause any

increase. I am a bit gun shy on the chemo. Any suggestions? I am currently being treated at city of hope, irvine campus.

Lupron Question.
Psa
was 19 in Sept and dropped to 1.26 as of Jan. 11 2024. Since going on Abi when I get a monthly Lupron shot I have trouble sleeping that first night and mild flu like symptoms the next day. I have agreed to try the 3 month injection but am concerned the immediate se’s may be worse.

treatment or no treatment?
To date I have elected not to be treated but instead have been monitoring my

Medical OR chemical castration

increasing monthly so I will try other chemicals. I want to remain in good shape until my number is up. My question is should I get medical castration to reduce the side effect of extended ADT injections or continue with the ADT. Does medical have less side effects?

Almost 3 years on ADT + Erleada
then switched to lupron and Erleada after a few months since I had a reaction to Abi after a couple of months .Erleada also gave me rash in the beginning so cut down to half dose which I have been on for the last 2 years.I get tested every 3 months and Scan yearly and so far mets have resolved and

Recently diagnosed and trying to work out what to do! All advice gratefully received.
I have a
psa
of 7.7. Gleason Score 3+4 = 7 one in 10 cores with a 2.5mm tumour length whoich is 20% of the core involved. 30% of cores showed cribreform pattern 4 glands. Area of concern on MRIs consistent on both MRIs and is where the positive biopsy was taken.
