Experience, thus far with Lupron and Abiraterone
I was diagnosed in May 2022 with PCa after my
PSA
was measured at 23.79. I have no other health issues. Following the determination that PCa was still within the prostrate capsule and had not spread to any other organ, I was placed on Lupron (6 monthly IM injections for 2 years).

2Nd Biopsy in 3+ years of Bone Met
But now his
psa
numbers are starting to climb. (From .04- .15 in the last 3 mos.) His onc. has requested another biopsy from a newly discovered lumbar lesion (via psma scan 2 weeks ago). We are curious, in what ways will this 2nd biopsy provide info. for next phase of treatment?

ADT SE’s
Current labs show undetectable
PSA
with a slow recovery on the T with the latest T result at 90 question is approx 6 months after the Lupron shot the SE started dissipating. Now I am experiencing the hot flashes again almost 8-9 later after the SE dissipated.

Accuracy of PSMA PET scan?
His
PSA
at dx was 13. In the "wives" group I'm a part of, I keep hearing about men who showed no spread on PSMA scan, but went for surgery and had positive lymph nodes, bladder neck invasion, perineum etc.

Meeting with our oncologist for castrate resistance metastatic prostate cancer
He told me his dad's numbers (testosterone 39 and
PSA
2.8) are still pretty low and if it wasn't for the pain we would keep him on the Xtandi for a bit longer while the
PSA
increased. After Zytega he said we don't have many good options, given my dad's age (81) and frailty.

Go-forward Treatment Plan
He just had a re-test few days ago and the
PSA
has remained at 0.48 over the month . With the rapid
PSA
rise, we had a PSMA PET scan done, and got the results today that there was [u]
uptake in the pelvic lymph node but nowhere else.

Any advice for a specialist in NYC to monitor my post-RP PSA?
So, still "undetectable" (ie <0.05), but I've been advised to get the
PSA
checked every 3 months. So, can anyone chime in to recommend a specialist to help me monitor my
PSA
? I assume a urologist is in order, unless my
PSA
does increase. Or maybe even just a GP?

Are Lymph nodes a filter?
When I took the ADT, my
PSA
went from 1000, down to 6, and all evidence of cancer disappeared from my lymph, pretty much immediately.

Starting the journey Interested in comments Gleason 5+4 Met to Lymph and Maybe ribs
I have just started this journey, and am deciding on treatments now. 60 years young with a Gleason 5+4
PSA
hovering around 7. My PSMA PET showed one lymph near pelvis for sure, and possibly a rib neat top on left side.

Treatment options
PSA
was 77 in Jan and dropped to 23 after starting ADT treatment and already feeling better with much less urinary symptoms. I was advised to start radiation treatment but doctor advised to wait for PSMA PET scan (Still waiting for scan).

Trimix with atrial fibrillation meds
My
PSA
has been negligible since then. I was discharged from the hospital yesterday, after an arterial flutter, my first heart problem ever. I was discharged with amiodarone (Pacerone), apixaban (Eliquis), losartan (Cozaar), and metropol succinate XL (Toprol XL), at least temporarily.

Help interpreting PMSA Pet scan
In the setting of rising
PSA
, the possibility of local/marginal recurrence of prostate carcinoma should be considered. Further evaluation with contrast-enhanced MRI with particular attention to this suspicious lesion is recommended. 2.
