I have my short synacthen test tomorrow morning! Please could someone let me know, do I need to fast from my thyroid medications for the test and instead take my thyroid medications after the short synacthen test?
Any help would be greatly appreciated!
Thank you so much

thank you so much for trying to help, I appreciate it! I haven’t received any information, but when I researched online it seemed to state I could eat food as normal. I was just a bit uncertain about the thyroid medication. I have done the short synacthen test in the past, but I cannot remember did I take my thyroid medication 😂!
I was reading “Stop the Thyroid Madness” book but I think I may have misinterpreted the advice! It seems to state to not take NDT until after morning and noon spits with cortisol salvia testing, it made me think, should I then also avoid taking my T4/T3 medications until after the short synacthen test? I attach photographs of what I read, after this message, please forgive me if I’ve misinterpreted the book?
I just don’t want to skew my short synacthen test and I don’t know if T3 does that? #confused thanks so much for your time and care 🥹🌺🌼🌸🌷🌻
I will try do some more reading tonight and if I find anything I’ll let you know as it’ll help us all in the group! Thank you for being there to care and help me 🥹🌸🌺🌻 sending big hugs
I think I may have found an indirect answer 🥳! According to “Stop The Thyroid Madness” book, apparently the ACTH short synacthen test only considers abnormal cortisol function to occur in the upper and lower 2% of the bell curve, where one spots Addison’s disease or Cushing’s syndrome (pg.55)! So one can have adrenal fatigue and dysfunction without falling in these far ends of the curve. As displayed in the picture (pg.99), it seems to be that T3 can raise low morning cortisol levels! I will follow the book and refrain from taking my thyroid medication before the ACTH short synacthen test to give a better chance of potentially spotting a sluggish ACTH response! I’ll try encourage my GP and Consultant to support me in saliva testing if the ACTH short synacthen test is normal!
Read the info on the Short Synacthen Test on pages 68 - 70.
It tells you the reasons for doing a test, the preparation for it, how the test should be carried out, and how to interpret the results (note the interpretation is based on the reference ranges at one particular hospital in London, but it should give you an idea of what to expect).
Advise you to take plenty of tissues, just in case. After a couple of minutes, I just suddenly burst into tears and couldn't stop - really weird. Can't remember my results but I was referred for insulin stress test and CT scan after that. Do NOT have one of those if you can avoid it. Made me ill for days.
