Can anyone please tell me where free t4 should be at for secondary hypothyroidism. Should it be in mid range to upper third? Mine was 1.01 in a range of 0.76 to 1.46. I feel very hypo.

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

is this how you do your tests

FT4: 1.01 pmol/l (Range 0.76 - 1.46)

Ft4 only 35.71% through range

So you need next dose increase in Levo

It 'should' be where it makes you well - and that's different for everyone - whether it's seconday hypo, primary or tertiary hypo. The aim of treatment is not to get any number to any particular place, but to make you well. Blood tests are just rough guides because the symptoms of over and under-medication can be the same. :)
