Help needed analysing test results

Had been feeling rubbish all winter getting every bug going. Gp agreed to a load of blood tests, diabetes apparently diagnosed but I have queries there. Have long thought thyroid or parathyroid disease which gran had but constantly told I’m within range for thyroid & they won’t test for parathyroid directly.

Can I post results here for analysis/opinion?

Yes please add results and ranges

Was test done early morning, this gives highest TSH

(TSH is all many GP’s look at or test)

For full Thyroid evaluation you need TSH, FT4 and FT3 tested 

Also both TPO and TG thyroid antibodies tested at least once 

Very important to test vitamin D, folate, ferritin and B12 too

Results I have are :

Sample collected 2/2/23

Tsh 1.84 mU/L

Free t4 12.4 pmol/L

Sample collected 16/2/23

tsh 1.84 mU/L

free t4 12.4 Pmol/L

I find it odd both days produced EXACTLY the same result.

No tpo or tg test. Or the others either.

I have fbc results though if that’s of any use?

not done early morning by normal standards but I have insomnia so it was early morning to my body if that makes sense? As in I had alarm to wake me up dressed & to drs

Please add reference ranges that come with your results. Ranges vary from lab to lab so we need yours to be able to interpret them. We know that your TSH is not suggesting either hypOthyroid or hypERthyroid but your FT4 can't be interpreted. We see many ranges here, eg

9-19

11-23

12-22

plus others.

So you could be mid-range with one of them or bottom of range with another.

Ft4 looks low

What’s the range on results? (Figures in brackets after result)

Just testing TSH and Ft4 is completely inadequate

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies 

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis. 

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease (Hashimoto’s or Ord’s thyroiditis)

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis 

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended on here that all thyroid blood tests early morning, ideally just before 9am

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

This is what you need for Full iron panel test for anaemia

Good vitamin levels are

Vitamin D at least over 80nmol

Serum B12 at least over 500

Active B12 at least over 70

Folate and ferritin at least half way through range

MCH 29.2 (27-32 U)

platelet count 211 (150-410 U)

neutrophils 4.5 (2-7 U)

Lymphocytes 2.8 (1.1-5 U)

Monocytes 0.6 (0.2-1 U)

eosinophils 0.19 (0.02-0.5 U)

basophils 0.1 (0-0.1 U)

Nucleated RBC 0

You are legally entitled to results and ranges

Results without ranges are pretty meaningless

FBC doesn’t show anything

Next step is get FULL thyroid and vitamin testing done

test early Monday or Tuesday morning around 9am

Are you currently taking any vitamin supplements

In theory I take 3000 vit d, vit c, b vits and magnesium

in reality I forget - a lot!

Guessing you’ll now tell me I need to be on top of this which I plan to be starting tonight.

did you test vitamin levels before starting supplements

Essential to retest at least annually

stop taking any supplements that contain biotin 5-7 days before any blood test as biotin can falsely affect test results as it’s used in many bits of lab testing equipment

3000 D is pretty excessive. Both GP and Endo want it at 2000 units max. It is nonsoluable so can build up in your body to toxic levels.

You cannot make a statement like that. 3000 D is not excessive but you do need to know what your level is before and during supplementation and adjust accordingly.

thank you, 3000 is not excessive I believe max is 5000? I was housebound with agoraphobia for several years, I’m not at the moment but still don’t get much sunlight. It was initially a cpns suggestion

Test vitamin D twice year

Once via full thyroid and vitamin testing

Then if not redoing with thyroid and vitamins use this test
