Hey1975,

Your GFR is very low for a someone of only 47 year old.

Thyroid hormone enables the kidneys to function at a normal rate and when deficient can decrease the glomeruler filter action rate, in which case creatinine usually raises but yours hasn't.

I don't know exactly what is going on but I've been hurting so bad it started on my right side or flank but its now across my lower mid back area. I actually thought I had a UTI or kidney infection but they keep saying nothing is wrong. No infection or anything else. My blood pressure is dropping really low after I take my dose of levothyroxine 25mcg and the pain is unbearable for several hours and then my blood pressure starts increasing and I end up with it going to high. I'm in tears and I don't even wanna take the levo anymore. It's happened since I was restarted on it so today makes my 4th day. But I can't go through this again. And I don't know if it's the levo doing it but I don't take anything else. I've read levothyroxine actually makes the kidneys and other things function better for hypo patients, so why is this happening?? Thank you for your time and information.

Oh how horrible becoming over medicated (in other post), and as you've found out it doesn't have to be with high amounts of thyroid hormone. I suffered physchosis for three days. I feel for you.

The GFR is notoriously difficult to accurately measure and with creatinine and albumin within range your result could well be a fluke after starting, stopping and raising thyroid hormone too quickly. Thyroid hormone drives everything and so low GFR is often due to a lower cardiac output and renal blood flow, hence alterations in BP.

The creatine bit comes in through tubular secretion but also release from muscles, hence the severe muscle pain and wastage seen in long term hypo people whose treatment is mismanaged. We end up with proteinuria and ironically thyroid hormones attach to these proteins so we end up losing even more. 

Although the liver is the main organ for conversion (& excretion) of thyroid hormones, the kidney offers a lesser amount and any impairment will alter thyroid physiology. Low T3 levels are the most common lab finding in CKD. Your most recent TFT's are unlikely to provide a true representation with all this other stuff going on but will provide a basis to prevent possible overdose again. What were your most recent TFT's?

 If I was you I would challenge your doctor why your GFR is indicating CKD and ask for further investigation, perhaps having urine measured if previous was blood or vice versa? Also ask for thyroid antibodies TPOAb and TGAb to be tested and other essential nutrients that haven’t been done, eg Vit B12, folate, Vit D and an iron panel (don't accept just ferritin with such skewed kidney results).

Here in UK when members experience uncooperation regarding essential blood testing  from their GP’s, they perform private labs and post results here. I hope you have something available to yourself also.

I'm just now seeing your reply. I'm still learning how to use this forum so thank you for the helpful tips on posting things. I wasn't sure exactly what how to do that. When I have a new issue or question do I just keep adding to my original post to keep everything in one place or do I post a new post & question? Also I'm not sure what TFT's are? I have asked about my GFR & they said it's probably from taking high amounts of nsaids and pain medication over the years and was told it's actually gone up I think I'm in the 60-64 GFR right now. I have been on 25 levo for 4 days and took 50mcg this morning and that really bad flank/back pain hasn't been there all day nor has my blood pressure fluctuated today, everything has been pretty steady. But I was advised to stay on the 25 levo so I'm really not sure what to do. 25 is a low dose I fully believe that going to 100 levo in 2 weeks is what sent me over the edge (hyper) last time. The only issue right now is I feel a little nervousness but nowhere near like before. That was HORRIBLE!! So I've gotta decide what to do before my next dose bc I cannot be flip flopping around with this stuff. I'm trying to see another doctor and have all the test the thyroid panels and the antibodies test but so far every one only wants TSH levels. Ive seen thyroid test prices and prices for testing my vitamin deficiency but I'm recently on my own and don't have any resources to get the test done on my own. And I don't have health insurance so everything I do is cash. I've spent every penny I had when I ended up (hyper) trying to get that taken care of. I'm just very overwhelmed and I don't have any kind of support system or help. It's been y'all helping me and I'm so thankful. Thanks again for your time and help. Enjoy your day/night. God bless.

Also, I'm supposed to start new COPD meds that should help me breathe better and get more oxygen. And I'm trying to quit smoking while my life has been turned upside down so that's another possible GFR explanation and my up and down blood pressure but not completely positive.

TFT’s is a term for ‘thyroid function tests’.

Yes, smoking is very influential within thyroid physiology and stopping can instigate big changes, even exposing a thyroid function problem that wasn’t evident before, ie stop smoking and get thyroid issues! I wrote a recent reply to another member here …   healthunlocked.com/thyroidu...

Adequate thyroid hormone levels are essential for good cardio vascular function to prevent conditions such as hypoxia, diaphragmatic dysfunction and myopathies, etc. Research evidences a direct correlation between inspiratory/expiratory muscle strength and the degree of hypothyroidism, which improves once thyroid hormones are optimised. If you wish to read more about the T3 & heart connection, a member called tattybogle has written some excellent posts all evidenced with credible research papers. 

Both high cortisol and low thyroid hormones will be influencing energy levels as a result of breakdown of good glycogen function required for mitochondria, and reducing our capacity for exercise. As thyroid hormone levels also change during systemic illnesses, it becomes yet another vicious circle. 

Regarding your posts, yes it’s good to keep things tidy in one post if you are receiving sufficient comment from members but sometimes when a post has run a while and more issue discussions are developing, starting a new post might invite other more varied feedback, ie it’s good to receive balanced opinions as non of us are medically trained but just share our experiences/knowlege. Do not worry about making mistakes as you are brave to post and especially when you feel so unwell. Members here are incredibly supportive as we’ve all been terribly let down one way or another 😊.

I just had a look at the other post you mentioned about smoking and that's very interesting. My COPD has never caused me much issues. But when I was over medicated and went hyper things started happening pretty quickly with my breathing. I started having low oxygen and from there it just got worse. On top of the high blood pressure and horrible anxiety from too much levothyroxine. I actually thought all my issues were the levothyroxine and thyroid. But it turns out it was both my COPD & Thyroid. So I'm trying to put down these cigarettes for good!! When your struggling to get air it's pretty scary! I'm mentally, physically and emotionally exhausted. I lost my mama when she was only 50 years old. She smoked her entire life & she decided 6 months before she passed to quit smoking. It was as if when she stopped smoking her body just started shutting down or something. And I feel like something similar is happening to me. And that causes me some anxiety. I was pretty much a chain smoker but never smoked in the he house. I have had bad experiences with nicotine patches or nicotine products so it's either cold turkey or less and less. Today in total I smoked 2 cigarettes. My kids are just shocked that I'm doing so well. But the truth is it's extremely HARD! I think it's the fear of not having air that's driving me to continue & I truly want a better life! There's so many things I still want & need to do in my life.
