ESR is an inflammation marker. It doesn’t not specifically diagnose a suspected problem but can be present with a number of conditions - including autoimmune.

Most primary hypothyroidism is due to Autoimmune thyroiditis (known as Hashimoto’s) So high ESR may be related to it that but it’s not related to the function tests of the thyroid.
