Completely Confused!!

I have recently been diagnosed with hypothyroidism and have numbers that have me totally baffled!! Can someone explain the following?

(*Note that the standard range is:

0.450 - 4.5)

9/4/20 3.24

7/28/20 12.030

6/11/20 19.810

7/30/19 0.406

I am currently on 0.088 mcg of Levothyroxine and am being seen by my primary care physician. I have been referred to an endocrinologist and am scheduled to be seen at the end of November. What is going on with these crazy numbers and should I keep my scheduled appointment with the specialist?

Just testing TSH is completely inadequate

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also EXTREMELY important to test vitamin D, folate, ferritin and B12

Low vitamin levels are extremely common, especially if you have autoimmune thyroid disease (Hashimoto's) diagnosed by raised Thyroid antibodies

Ask GP/endocrinologist to test vitamin levels and thyroid antibodies

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Why was dose reduced

Most recent TSH of 3.24 shows you need dose increase

On levothyroxine most people will have TSH under 1 when adequately treated

The aim of levothyroxine is to increase dose upwards until Ft4 is in top third of range and Ft3 at least half way through range (regardless of where TSH is) ...important to have optimal vitamin levels too as this helps reduce symptoms and improve how levothyroxine works

Thank you so much for your reply (again). 🙂 Yes, I was on 100 mcg of levothyroxine and it was reduced due to severe gastrointestinal issues. I am currently taking 2000 iu of vitamin d3 once daily, 500 mg of krill oil daily and a women’s multivitamin (vague, I know), all over the counter. I have been very careful about when and how I am taking the levothyroxine, hopefully helping me to get the best results and most accurate test results. If I’m understanding what I’ve read, you are saying that my results are showing I’m “in range” but not in “optimal range”. It seems that I still may have a journey yet to go and this has only just begun. From the information shared, I am gathering it is important to keep the appointment I have scheduled with the endocrinologist in November. The last message I received from my primary care doctor was “follow up in four months.” 🙃 Thank you again for the information you’ve shared and of your time. I welcome your thoughts and suggestions. Gut issues are EXTREMELY common when under medicated. Low Ft3 leads to poor gut function, low stomach acid and low vitamins

Gluten intolerance and dairy intolerance common too

Low stomach acid can be very a common hypothyroid issue

Good morning!!! Thank you again for your response and thank you for all of the informative links. I definitely have some reading and some self educating to do! There are so many variables and things to take into consideration. I have plenty of time to do my homework before my appointment in November. Thank you again!!
