Your results are slightly below normal reference range for that time of day. Around 8-9am cortisol should be nearer 500nmol/Ls. I would hope that your GP will refer you to an Endocrinologist for a short synacthen test to see how well your adrenal glands respond to the artificial ACTH injection. They take a base line cortisol & then following the injection they see if your cortisol reaches 450nmol/Ls after 30 mins, if it does then your adrenal glands works well, if it doesn't then it's likely you have Addison's disease. However it's important to have an ACTH blood test done before they do the injection, this needs to go on ice straight away. Many Endo departments don't do this test even though it will show if you have secondary adrenal insufficiency meaning that means the issues is with the pituitary gland. Also many Endo departments aren't conversant in pituitary/adrenal issues so it's important to go tone that is.

I also recently did a 24 hour urine cortisol test through my cardiologist, im guessing this was fine as last visit he didn't say anything was wrong?

24 hr urine isn't great for testing low cortisol as it only overflows into the urine when it is at high levels.
