What does your wife's doctor say? Is she diagnosed with a thyroid problem? Is she on any medications?

A TSH of 4.1 may not be the optimum for pregnancy. But it's not good to go by TSH alone. Ideally the levels of FT4 and FT3 in the blood should also be measured to give a fuller picture. Also, if there is concern that the thyroid may be underactive, thyroid antibodies should be measured to see if there is an autoimmune issue involved.
