When on 125 mcg ,so out of breath on excertion.When reduced to 112.5 mcg daily shortness of breath improved but pains in arms and feeling low worse.Been on 112.5 for 14 days now.Why is this happening ?

I think you need a Free T4 and Free T3 blood test. Both should be optimum and I'll give you a link to peruse:-

Essential to test vitamin D, folate, ferritin and B12 as well
Low vitamin levels are extremely common especially with Hashimoto's
Do you have Hashimoto's? If so are you on strictly gluten free diet or tried it
Hi slowdragon,yes I have Hashimotos,but d 38 so on vit d.
So what about ferritin, B12 and folate
Low iron or ferritin can cause breathlessness
How much vitamin D are you taking and have you retested since started supplementing?
Aiming to improve to around 100nmol. Vitamin D mouth spray by Better You is good as avoids poor gut function. Suggest you supplement for 2-3 months and retest. It's trial and error what dose each person needs.
Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Are you on strictly gluten free diet?
According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps significantly. Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)
Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies
Ideally ask GP for coeliac blood test first.
