If your TSH was high when you were diagnosed you have primary hypothyroidism (thyroid failure). If TSH was low-normal but FT4 and FT3 were low at diagnosis you have central hypothyroidism due to pituitary or hypothalmic dysfunction.

Do you know or remember if you have ever had your thyroid antibodies tested? If the result was ever high then you definitely have Hashimoto's which is autoimmune thyroid.
Phone the receptionist at your GP surgery and politely ask for copies of your recent Thyroid blood tests. You are entitled to have your own results, but it may take a day or two as they have to get a GP to OK it, before you go in & pick the results up.
Also have you had recent tests of vitamin D, folate, ferritin and B12? Again you need the actual results and the ranges (figures in brackets after each result). Very common for these to be low, then we struggle to use thyroid hormones.
If these vitamins and antibodies have not been done & you continue to feel unwell, ask GP if they could be tested. You can say you asked advice on the NHS recommended thyroid support group.
