i am on 75mgs of thyroxine(synthetic) my boods indicate the levels are ok tsh is 0.01 but i get bouts of fatigue this is over several years

TSH at 0.01 might indicate that you're slightly overdosed and that could be the cause of the fatigue. With such a low TSH, it's usual for the FT4 to also be tested, so the FT4 reading would give a clearer picture.

You may still need an increase in medication. Some people need to have a suppressed TSH.

But some need a higher dose of levothyroxine to suppress serum TSH and then the serum-free T4 concentration will be elevated at around 24-28pmol/l.

This ‘exogenous subclinical hyperthyroidism’ is not dangerous as long as serum T3 is unequivocally normal – that is, serum total around T3 1.7nmol/l (reference range 1.0-2.2nmol/l).

Even while taking the slightly higher dose of levothyroxine a handful of patients continue to complain that a sense of wellbeing has not been restored. A trial of levothyroxine and tri-iodothyronine is not unreasonable. The dose of levothyroxine should be reduced by 50µg daily and tri iodothyronine in a dose of 10µg (half a tablet) daily added.

My T4 is around 24pmol /i so "is normal" I am going on holiday so can't get to see consultant prior I think I will increase thyroxine to 100mg daily see how it goes the fatigue is overwelming right now and i fall asleep at the drop of a hat when I sit down.I hope i am doing right thing.

Your FT3 would give you a more accurate picture, especially if it is in the lower part of the range. If it's over the range, you could be slightly overdosed. If your FT3 is very low you could have the useless reverse T3 being stored in receptors that FT3 should be using.

Hi Geffers,

To be honest how I feel re low thyroid function symptoms has never bourne any relation to the "glowing" blood test results. I'm sure I've read that levo can just pool in the blood, give a great reading, but very little arrives in the cells. Dr S is always telling me that those blood tests have never been validated. It's how you feel that counts.
