Further treatment - Update
I started
Xtandi
in April 2019 and switched to Nubeqa December 2021. The PSA started to rise in fall 2021 after being steady at 0.1/0.2 for a long time. It seems now the cancer is castration resistant. I had my last PET scan in May without any findings.

Docetaxel/Taxotere. 2nd time in 3 years. It's a no go this time. No benefit after 3 infusions.

is viable" after the Zytiga I've ran thru). Meantime a nice Christmas break from the chemo crashes, some recovery before hitting "IT" again.

Luu-177 available in Southern California? It's very limited in Phoenix
My husband is stable on Arberaterone and prednisone but has already used failed on
Xtandi
and docetoxyl. Phoenix will keep us on the list but I'm wondering if I should start investigating other alternatives. The doctor seemed to think it was a good idea.....

Getting Slightly Concerned
In November having been on ADT for 4 months and
Xtandi
for just 1 month, things were looking pretty good – PSA dropped from 13 at dx to 0.33 (although I was getting a little more pain from some bone mets).

just over 3 months. Just got first psa since this regimen 0.12. Radiation IMRT to begin in a week. Is this psa okay, will it decrease after radiation?

Is it too late for Triplet Therapy? (4 months on Elligard already.).
Also, old MO took 3 weeks to get the
Xtandi
Rx she wanted me to start processed. Pharmacy just called and left message about shipping instructions.

Went to tumor board and treatment suggestion was Lupron -casodex 4 weeks then Lupron -
Xtandi
plus 8 weeks Radiate to prostate and lymph. PSA dropped to .006 and remains that level after 1 year - still on Lupron -160 mg

everyday . Side effects arw occasional hot flash and shoulder pain .

Chemo/Pluvicto or Clinical Trial?
I'm also considering a clinical trial at NY Columbia Presbyterian which is a phase 2 trial evaluating Lutetium 177Lu-PSMA-I&T compared to
Xtandi
(control). If I get the
Xtandi
, I heard it probably won't work since Zytiga has failed. Are there any suggestions for the next treatment?

Pluvicto / Lu-177 - Do it now or later?
I am a metastatic castration resistant prostate cancer patient having undergone most SOC (RP, SR, docetaxel, ADT,
Xtandi
, Zytiga, Oliparib) steps. Now, like others, faced with very limited remaining SOC options: another round of the dreaded chemo like docetaxel or Lu-177 / Pluvicto.

Erratic PSA on jevannta
Plvitco would be next but want to get as much mileage from jevtanna, do not want to run out of options yet, last chemo was 2018, with lupron since then 1 year and half on Zigtea and 3 months on
xtandi
that failed .

Has anyone returned to the first line of HT after second line failed?
Question 1: I would appreciate anyone who can share their experiences about returning to the first line of HT (Casadox) after the second line (
Xtandi
) failed. If so, how effective it is. I was thinking that doing ADT alternatively can stop castration resistence.

After 11 years, dad has started chemo for the first time
Been on Zoladex (for life), Zytiga,
Xtandi
, etc. Now that his PSA is very slowly increasing (been going up since 2019 from below 0.30 to now at 2.0+), his oncologist recommended we enroll him in a Lu-177 clinical trial.

Nothings worked. Help please

after 1 month psa down from 150 to 115, after second month, now, psa 210. Hubby comes out of hospital today after 9 weeks. Had a fracture on the t10 vertebrae, operation to fix it 3 weeks ago. Going for radiotherapy on spine Mets on 29th Dec. On top of all this shite, he has covid.

Why we add an AART drug to ADT?
In response to a post asking if adding enzalutamide to ADT is beneficial:
Xtandi
would increase T levels if not simultaneously using an LHRH ADT. It blocks AR in hypothalamus leading to perception of deficit and increased LH and testicular testosterone.
