I hope this finds each and every one of you well. My doctor has prescribed Xtandi as my next line of therapy; can anyone who has used this medicine provide their views on it.

Please feel free to field your questions, but you can get some great foundation from those other posts. Xtandi Lupron have been miracle drugs for me.

Hey Brother, in my case after Abiraterone failed after 9 months in 2020, Xtandi failed in two months with PSA going from 5.2 to 9.2 in about a month. Had 6 more Taxotere sessions (had 15 in 2015) and that failed. About to get my 18th Cabazitaxel next Friday and looking at MRI guided IMRT probs next year.
Been on Lupron since 01/2015 and am Castrate Resistant - Duh

Me too I've been on 6 mth shots Lupron for 7-8 yrs now and Xtandi for over 2 yrs. Im staying undetectable for now. Yes, the side effects suck but hey I am alive, and the Lord has blessed me with the best yrs. of my life here in Montana with my wife. Go to the gym a couple days a week to work all your muscle groups. I'm only 67 and never thought I'd make it here alive. Give it to God. read Job in the Bible and know that in the end it will all be ok
How is your PSA at this point and how long did it take to bring it down?
Hi, PSA pre Cabazitaxel was 5.5 on 09/24/21 and the PSA from 10/25/22 is 6. The nadir from diagnosis was 0.1 in the Summer of 2017 , at diagnosis in 12/2015 it was 840.2. Prostate still present as there were Mets in 2015 to L ureter lymph nodes.
So, prognosis wise, the long slow drop from 840.2 in 12/2014 2014 to 0,1 18 months later may have been a good prognostic indicator.
But, the PSMA from 10/05/22 showed huge avidity in L prostate with an SUV at 87.5. Yikes
IMPRESSION:
1. Large zone of tracer avidity throughout the prostate gland
extending into the left inferolateral periprosthetic tissues and
perineum with max SUV up to 87.5 (very high PSMA expression)
consistent with locally aggressive prostate cancer.
2. No regional or distant tracer avid metastatic disease

I had dramatic results for six months and then it failed. For others it can work longer. I think its success varies enormously between patients but my scans showed improvements throughout so I think it is worth having even if its success is limited. The side effects are manageable but try to keep fit to manage fatigue.
I've had a good response to Xtandi so far, working for over 2 years. Good luck.
It's known to work.My advice would be.
Every one is different.
GO FOR IT...
try it out.
Check in to forum for any side effects 'if' you get them.
Make decisions on side effects as and when you get them.
I have my own experience of enzalutamide and it wasn't for me, I also know I may need revisit it at some point?

Hi Larry, did you ever have Zytiga or started Xtandi first? It would be great to know that, as my PSA starting climbing after 3.5 years Zytiga, I was wondering if switch to Xtandi would work for me
I believe conventional wisdom is that once one 2nd gen fails you are assumed to have cross resistance across all of them.
I've seen people on this forum who've had success with zytiga-> xtandi sequence.
no my oncologist put me on Xtandi as soon as I finished chemo.
sometimes you get resensitized to xtandi after chemo

Interesting. Is there is a reason why this isn't used widely for people for who these meds stop working.
I was on Xtandi for 6 years. I do think, in the long run (after years) that brain fog develops from fighting the tiredness side effect. It kept my PSA around 1.0 or below for most of that time.
I had bicalutamide (Casodex) as initial drug (with Lupron, of course) and that worked for about a year. After that I started Xtandi and it worked very well until it didn't.
Got for it! And exercise and get plenty of rest.
